摘　　要：目的研究腹腔镜保留肾单位肾肿瘤切除术(RPNSS)治疗肾细胞癌(RCC)患者的临床效果。方法将80例RCC患者根据手术方案不同分为RPNSS组与腹腔镜根治性肾切除术(RPRN)组,各40例。RPNSS组采用RPNSS治疗,RPRN组采用RPRN治疗。比较两组的治疗效果。结果RPNSS组的住院时间短于RPRN组(P<0.05)。术后1 d,RPNSS组的Scr水平低于RPRN组,eGFR水平高于RPRN组(P<0.05)。术后1、6个月,RPNSS组的KPS评分高于RPRN组(P<0.05)。RPNSS组的并发症总发生率低于RPRN组(P<0.05)。随访1年,两组的复发率比较,差异无统计学意义(P>0.05)。结论RPNSS治疗RCC患者可显著保护肾功能,缩短住院时间,且具有复发少、安全性高的特点。Objective To study the clinical effect of retroperitoneoscopic nephron sparing surgery(RPNSS)in the treatment of patients with renal cell carcinoma(RCC).Methods Eighty patients with RCC were divided into RPNSS group and retroperitoneoscopic radical nephrectomy(RPRN)group according to the different operation plans,with 40 cases in each group.The RPNSS group was treated with RPNSS,while RPRN group was treated with RPRN.The therapeutic effects of the two groups were compared.Results The hospital stay in the RPNSS group was shorter than that in the RPRN group(P<0.05).On the first day after operation,the level of Scr in the RPNSS group was lower than that in the RPRN group,and the level of eGFR was higher than that in the RPRN group(P<0.05).On 1 and 6 months after operation,the KPS scores in the RPNSS group were higher than those in the RPRN group(P<0.05).The total incidence of complications in the RPNSS group was lower than that in the RPRN group(P<0.05).After 1-year follow-up,there was no significant difference in the recurrence rate between the two groups(P>0.05).Conclusion RPNSS in the treatment of RCC patients can significantly protect renal function,shorten the hospital stay,and it has the characteristics of less recurrence and high safety.
