摘 要:目的探讨DNMT1和DNMT3a在进展期直肠癌新辅助治疗中的表达及临床意义。方法选取50例行新辅助治疗的进展期直肠癌患者,应用免疫组化法检测新辅助放化疗前后DNMT1和DNMT3a在直肠癌组织中的表达和临床意义,分析二者在直肠癌组织中的相关性,并根据肿瘤消退学分级(TRG)评估新辅助治疗的有效性。结果新辅助治疗前DNMT1和DNMT3a在直肠癌组中阳性表达率分别为84.00%(42/50)和78.00%(39/50),二者阳性表达与直肠癌分化程度、cTNM分期、淋巴结转移及肝转移相关(P<0.05);新辅助治疗后DNMT1和DNMT3a在直肠癌组中阳性表达率分别为36.00%(18/50)和30.00%(15/50),与化疗前二者的阳性率相比差异有统计学意义(P<0.05)。TRG分级:0级2例(4.00%),1级22例(44.00%),2级19例(38.00%),3级7例(14.00%)。相关性分析表明:二者在直肠癌组织中的表达呈明显正相关性(r=0.455,P<0.05)。结论DNMT1和DNMT3a在进展期直肠癌组织中的阳性表达与肿瘤分化程度、TNM分期等病理特征相关,且二者呈明显正相关性,DNMT1和DNMT3a可作为进展期直肠癌新辅助治疗疗效评估的重要分子生物学指标。Objective To investigate the expression and clinical significance of DNMT1 and DNMT3a in neoadjuvant therapy of advanced rectal cancer.Methods Fifty patients with advanced rectal cancer undergoing neoadjuvant therapy were selected.Immunohistochemistry was used to detect the expression and clinical significance of DNMT1 and DNMT3a in rectal cancer before and after neoadjuvant radiochemotherapy.Analyze their relevance and evaluate the effectiveness of neoadjuvant therapy based on the tumor regression grade(TRG).Results The positive expression rates of DNMT1 and DNMT3a in the rectal cancer group before neoadjuvant therapy were 84.00%(42/50)and 78.00%(39/50),respectively.The positive expression and differentiation of the two,cTNM stage,lymph node metastasis and liver Metastasis-related(P<0.05);the positive expression rates of DNMT1 and DNMT3a in the rectal cancer group after neoadjuvant therapy were 36.00%(18/50)and 30.00%(15/50)respectively,which was similar to the positive rates of the two before chemotherapy.The difference was statistically significant(P<0.05);TRG classification:Grade 02 cases(4.00%),Grade 122 cases(44.00%),Grade 219 cases(38.00%),Grade 37 cases(14.00%);Correlation analysis showed that the expression of the two in rectal cancer tissue was significantly positively correlated(r=0.455,P<0.05).Conclusion The positive expressions of DNMT1 and DNMT3a in advanced rectal cancer tissue are related to the tumor differentiation,TNM stage and other pathological features,and there is a significant positive correlation between the two.DNMT1 and DNMT3a can be used as important molecular biological indicators neoadjuvant therapy for advanced rectal cancer.
