摘　　要：目的探析血管内皮生长因子(vascular endothelial growth factor,VEGF)、基质金属蛋白酶-9(Matrix metalloproteinase-9,MMP-9)及基质金属蛋白酶抑制剂-1(tissue inhibitor of metalloproteinase-1,TIMP-1)在视网膜母细胞瘤(Retinoblastoma,Rb)患儿中的临床应用价值。方法免疫组化法检测2007年4月至2018年4月间在我院接受诊断治疗并手术切除的47例Rb组织标本内的VEGF、MMP-9及TIMP-1水平。分析VEGF、MMP-9及TIMP-1的阳性表达率及其与Rb肿瘤组织分化及浸润的关系。结果Rb患儿TIMP-1的阳性表达率明显低于VEGF、MMP-9(P<0.05)。分化型Rb组织内VEGF、MMP-9的阳性表达率明显低于未分化型(P<0.05)。有视神经浸润的VEGF及MMP-9阳性率明显高于无视神经浸润者(P<0.05);分化型与未分化型、视神经浸润者与无视神经浸润者的TIMP-1阳性率无明显统计学差异(P>0.05)。Rb组织内TIMP-1与VEGF、MMP-9表达无显著相关性(r=0.234,0.147;P>0.05);VEGF与MMP-9存在显著正相关性(r=0.561;P<0.05)。结论高表达状的VEGF与MMP-9在Rb患儿肿瘤细胞的分化浸润及其新生血管的形成过程中发挥了重要作用。Objective To explore the clinical value of the detections of vascular endothelial growth factor(VEGF),matrix metalloproteinase-9(MMP-9)and tissue inhibitor of metalloproteinase-1(TIMP-1)in children with retinoblastoma(RB).Methods The levels of VEGF,MMP-9 and TIMP-1 were measured by immunohistochemistry in 47 RB tissues from April 2007 to April 2018.The positive expression rate of VEGF,MMP-9 and TIMP-1 and their relationship with the differentiation and infiltration of Rb tumor were analyzed.Results The positive expression rate of TIMP-1 in RB was significantly lower than that of VEGF and MMP-9(P<0.05).The positive expression rate of VEGF and MMP-9 in differentiated RB was significantly lower than that in undifferentiated Rb(P<0.05).The positive rate of VEGF and MMP-9 in the patients with optic nerve infiltration was significantly higher than that in the patients without optic nerve infiltration(P<0.05).There was no significant difference in the positive rate of TIMP-1 between the patients with and without optic nerve infiltration(P>0.05).There was no significant correlation between TIMP-1 and VEGF,MMP-9 expression in Rb(r=0.234,0.147,P>0.05).There was significant positive correlation between VEGF and MMP-9(r=0.561,P<0.05).Conclusion High expression VEGF and MMP-9 play an important role in the differentiation,infiltration and neovascularization of Rb children's tumor cells.
