摘　　要：目的探讨鸦胆子油软胶囊口服联合芬太尼透皮贴剂治疗老年骨转移癌疼痛疗效及生活质量影响。方法选择我院肿瘤内科确诊为老年骨转移癌150例,采用随机数字法分为观察组1、观察组2和对照组,各50例。对照组给予布洛芬缓释胶囊口服,观察组1给予芬太尼透皮贴剂治疗;观察组2给予鸦胆子油软胶囊联合芬太尼透皮贴剂治疗。疗程均15 d,比较三组疼痛评分、临床有效率及生活质量。结果三组疼痛评分比较治疗前无显著差异(F=0.17,P>0.05);治疗第7天有显著差异(F=8.56,P<0.01),但观察组2较观察组1无显著差异(t=1.27,P>0.05);治疗第14天疼痛评分比较,差异有显著性(F=14.84,P<0.01),但观察组2与观察组1无显著差异(t=1.32,P>0.05);三组患者治疗15 d后疗效比较,差异有显著性(χ^2=120.26,P<0.01);治疗第7天生活质量比较,差异有显著性(F=199.80,P<0.01);治疗第14天生活质量比较,差异有显著性(F=394.40,P<0.01);治疗第14天观察组2生活质量较观察组1有显著提高(t=2.32,P<0.05)。结论芬太尼透皮贴剂可显著减轻骨转移癌疼痛,鸦胆子油软胶囊口服可提高骨转移癌患者生活质量。Objective To investigate the effect of Brucea Javanica oil soft capsule combined with fentanyl transdermal patch on the efficacy and quality of life of elderly patients with bone bone metastatic cancer.Methods A total of 150 elderly patients with bone metastatic cancer were enrolled in the department of oncology of our hospital,and were randomly divided into observation group 1,observation group 2 and control group,with 50 cases in each group.The control group was given ibuprofen sustained release capsule orally.The observation group 1 was given fentanyl transdermal patch treatment.The observation group 2 was treated with Brucea javanica oil soft capsule combined with fentanyl transdermal patch.The course of treatment was 15 days.The pain scores,clinical effectiveness,and quality of life between three groups were compared.Results There were no significant differences in the pain scores between the three groups before treatment(F=0.17,P>0.05).The pain scores on the 7th day of treatment were significantly different(F=8.56,P<0.01),but there was no significant difference between the observation group 2 and observation group 1(t=1.27,P>0.05).There was significant difference in pain score on the 14th day of treatment(F=14.84,P<0.01),while there was no significant difference between the observation group 2 and observation group 1(t=1.32,P>0.05).The difference in the efficacy of the three groups after treatment for 15 days was significant(χ^2=120.26,P<0.01).The difference in qual ity of life on the 7th day of treatment was significant(F=199.80,P<0.01).There was significant difference in quality of life on the 14th day of treatment(F=394.40,P<0.01).On the 14th day of treatment,the quality of life observation group 2 was significantly higher than that of the observation group 1(t=2.32,P<0.05).Conclusion Fentanyl transdermal patch can significantly reduce the pain of bone metastasis cancer.Brucea javanica oil soft capsule oral administration can improve the quality of life of patients with bone metastasis cancer.
