白内障吸除、后囊环形撕囊、前部玻璃体切除及人工晶状体Ⅰ期植入术治疗小儿单眼先天性白内障的效果

摘　　要：目的探讨白内障吸除、后囊环形撕囊、前部玻璃体切除及人工晶状体Ⅰ期植入术治疗小儿单眼先天性白内障的效果。方法2017年1~12月将66例发育性或外伤性白内障患儿80只眼随机分为A组和B组,每组40只眼,每组再分为1组和2组,各有20只眼,A组进行白内障吸除手术联合人工晶状体Ⅰ期植入术,B组在A组治疗基础上采用后囊环形撕囊、前部玻璃体切除手术。观察并分析术后视力、视轴混浊及可能发生的并发症。结果B组水肿、积血、渗液、眼压升高等并发症均高于A组,差异有统计学意义(P<0.05)。结论白内障吸除、后囊环形撕囊、前部玻璃体切除及人工晶状体Ⅰ期植入术联合应用治疗小儿单眼先天性白内障有效可靠。Objective To analyze the effect of cataract extraction,posterior continuous curvilinear capsulorhexis,anterior vitrectomy and stageⅠintraocular lens implantation in the treatment of congenital monocular cataract in children.Methods A total of 80 eyes of 66 children with developmental or traumatic cataract from January to December 2017 were divided into group A and group B,with 40 eyes in each group,which was further divided into group 1 and group 2,with 20 eyes in each group.Group A was treated with cataract extraction combined with stageⅠintraocular lens implantation and group B was treated with posterior continuous curvilinear capsulorhexis and anterior vitrectomy on the basis of group A.The postoperative visual acuity,visual axis opacity,and possible complications were observed and analyzed.Results The incidences of edema,hematocele,exudate and elevated intraocular pressure in group B were all higher than those in group A,with statistically significant differences(P<0.05).Conclusion The combined application of cataract extraction,posterior continuous curvilinear capsulorhexis,anterior vitrectomy and stage I intraocular lens implantation is effective and reliable in the treatment of congenital monocular cataract in children。
