摘　　要：目的探讨生存素(Survivin)、黏蛋白2(MUC2)、黏蛋白5(MUC5)在结直肠癌(CRC)中的表达,以及与CRC患者的临床病理特征之间的关系。方法回顾性的分析80例结直肠癌患者,并选取同期20例正常结肠黏膜作为对照组。采用免疫组织化学及Western blot方法检测Survivin、MUC2和MUC5在健康人和CRC组织中的表达水平。并分析Survivin在CRC中的表达及其与MUC2、MUC5、Ki-67的关系和临床病理特征。结果免疫组化和印迹的结果表明Survivin在CRC组织中高表达,在正常结肠组织中很少表达。此外,Survivin和MUC5的过度表达与CRC淋巴结转移、细胞分化不良、肿瘤晚期和预后差密切相关。相反,MUC2的低表达与CRC淋巴结转移、细胞分化不良和肿瘤晚期显著相关。低表达Survivin和MUC5的患者的2年生存率高于高表达survivin和MUC5的患者,而具有高表达MUC2水平的患者的2年生存率高于低表达MUC2水平的患者。结论Survivin、MUC5的高表达以及MUC2的低表达与肿瘤进展显著相关;Survivin、MUC5高表达和MUC2低表达的CRC患者淋巴结转移率和晚期肿瘤的发生率显著增加。Survivin、MUC5和MUC2可协同参与结直肠癌的早期诊断和临床特征判断。Objective To investigate the association between Survivin,mucin 2(MUC2)and MUC5 expression,and the clinicopathological features of colorectal cancer(CRC).Methods The expression levels of Survivin,MUC2 and MUC5 in healthy volunteers and CRC tissues were detected by immunohistochemistry and Western blot.The expression of Survivin in CRC and its relationship with MUC2,MUC5,Ki-67 and clinicopathological features were analyzed.Results A retrospective analysis was performed on 80 patients with colorectal cancer,and 20 normal colonic mucosa were selected as the control grouThe immunohistochemistry and western blotting results demonstrated that Survivin was highly expressed in CRC tissues and rarely expressed in normal colon tissues.Moreover,the overexpression of Survivin and MUC5 was strongly associated with lymph node metastasis,poor cellular differentiation,advanced tumor stage and a poor prognosis in CRC.By contrast,low expression of MUC2 was signifcantly associated with lymph node metastasis,poor cellular differentiation and an advanced tumor stage in CRC.The 2-year survival rate of those whose tumors expressed low levels of survivin and MUC5 was higher than that of patients with high expression levels of survivin and MUC5.By comparison,the 2-year survival rate of those with tumors expressing high levels of MUC2 was higher than that of patients whose tumors expressed low levels of MUC2.Conclusion The high expression of Survivin and MUC5 and the low expression of MUC2 were significantly correlated with tumor progression,and the lymph node metastasis rate and the incidence of advanced tumor were significantly increased in patients with high expression of Survivin,MUC5 and low expression of MUC2.Survivin,MUC5 and MUC2 can be involved in the early diagnosis and clinical characteristics of colorectal cancer.However,the detailed mechanism of their differential expression in colorectal cancer needs to be further studied.
