白花蛇舌草注射液在中晚期肺腺癌治疗中的作用及对血清中miR-155的影响研究

摘　　要：目的探讨中晚期肺腺癌患者辅助应用白花蛇舌草注射液的临床效果,关注其对血清中miR-155的影响。方法方便选择该院2018年1—12月收治的82例中晚期肺腺癌患者,按入院顺序分为观察组(41例)和对照组(41例),对照组应用顺铂联合长春瑞滨治疗,观察组在上述基础上加用白花蛇舌草注射液进行治疗,治疗2个周期观察疗效,同时关注治疗前、后血清中微小RNA-155(miR-155)的表达水平。结果观察组共38例、对照组共38例完成治疗及观察。两组治疗总有效率﹙55.3%vs 28.9%﹚,差异有统计学意义(χ^2=5.398,P=0.020)。两组在骨髓抑制(26.3%vs 48.6%),(χ^2=3.996,P=0.046)及呕吐III和IV度(23.7%vs 55.3%),(χ^2=7.930,P=0.005)的发生率比较中有差异统计学意义(P<0.05)。治疗后两组血清中miR-155的表达均下降,但是观察组的下降值明显高于对照组(t=4.020,P=0.012)。结论中晚期肺腺患者在顺铂联合长春瑞滨治疗方案的基础上积极应用白花蛇舌草注射液,临床效果明显,可降低化疗的不良反应,并能有效下调血清中miR-155的含量,临床中可以积极应用。Objective To investigate the clinical effect of adjuvant application of Herba Hedyotis extract in patients with advanced lung adenocarcinoma,and to observe its effect on serum miR-155.Methods Convenient selection eighty-two patients with advanced lung adenocarcinoma from January to December 2018 were enrolled in our hospital.They were divided into observation group(41 cases)and control group(41 cases).The control group was treated with cisplatin combined with vinorelbine.The observation group was treated with Hedyotis diffusa injection on the above basis.The therapeutic effect was observed in two cycles,and the expression level of microRNA-155(miR-155)in the serum before and after treatment was also considered.Results A total of 38 patients in the observation group and 38 patients in the control group were treated and observed.The total effective rate of the two groups(55.3%vs 28.9%),the difference was statistically significant(χ^2=5.398,P=0.020).There was a statistically significant difference between the two groups in the incidence of myelosuppression(26.3%vs 48.6%),(χ^2=3.996,P=0.046)and vomiting III and IV(23.7%vs 55.3%),(χ^2=7.930,P=0.005).The expression of miR-155 in the serum of both groups decreased after treatment,but the decrease of the observation group was significantly higher than that of the control group(t=4.020,P=0.012).Conclusion The patients with advanced lung adenocarcinoma are actively applied to the treatment of Herba Hedyotis extract on the basis of cisplatin combined with vinorelbine treatment.The clinical effect is obvious,which can reduce the adverse reactions of chemotherapy and effectively down-regulate the content of miR-155 in serum.It can be actively applied in clinical practice.
