新型抗癫痫药物联合CPAP治疗癫痫合并阻塞性睡眠呼吸暂停综合征的疗效探讨

摘　　要：目的探讨新型抗癫痫药物联合CPAP治疗癫痫合并阻塞性睡眠呼吸暂停综合征的疗效。方法方便选择2017年4月—2018年9月该院收治的癫痫合并阻塞性睡眠呼吸暂停综合征患者72例,根据患者能否接受CPAP治疗,分为观察组36例,对照组36例,两组患者都给予拉莫三嗪与左乙拉西坦等新型抗癫痫药物治疗,观察组在使用新型抗癫痫药物治疗的同时使用CPAP联合治疗,观察两组治疗前后癫痫发作次数、呼吸暂停低通气指数与低通气总时间、脑电图改善效果。结果观察组与对照组相比,治疗后每周癫痫发作次数明显减少;治疗期间的呼吸暂停低通气指数明显降低,暂停呼吸低通气总时间明显减少。观察组的脑电图改善总有效率86.11%与对照组63.89%比较,差异有统计学意义(χ^2=5.071,P=0.009)。结论新型抗癫痫药物联合CPAP治疗癫痫合并阻塞性睡眠呼吸暂停综合征,其疗效显著,临床可充分根据患者能否耐受CPAP来合理选择治疗方式。Objective To explore the efficacy of a new antiepileptic drug combined with CPAP in the treatment of epilepsy with obstructive sleep apnea syndrome.Methods 72 patients with epilepsy complicated with obstructive sleep apnea syndrome admitted convenient to the hospital from April 2017 to September 2018 were enrolled.According to whether patients can receive CPAP,36 patients in the observation group and 36 patients in the control group.Patients were treated with new antiepileptic drugs such as lamotrigine and levetiracetam.The observation group was treated with CPAP in combination with new antiepileptic drugs.The number of epileptic seizures,apnea hypopnea index and total duration of hypopnea were observed before and after treatment.Results Compared with the control group,the number of seizures per week was significantly reduced in the observation group.The apnea hypopnea index was significantly reduced during the treatment period,and the total time to suspend respiratory hypoventilation was significantly reduced.The total effective rate of EEG improvement in the observation group was 86.11%compared with 63.89%in the control group.The difference was statistically significant(χ^2=5.071,P=0.009).Conclusion The new antiepileptic drug combined with CPAP in the treatment of epilepsy combined with obstructive sleep apnea syndrome has a significant effect,and the clinical treatment can be reasonably selected according to whether the patient can tolerate CPAP.
