摘 要:目的:研究血清S-100β与老年急性脑梗死患者非快速眼动期睡眠障碍及认知功能的关系。方法:纳入2015年5月至2019年5月于我院收治的216例老年急性脑梗死患者为对象,按是否出现非快速眼动期睡眠障碍分为A组(伴非快速眼动睡眠期睡眠障碍,112例)、B组(不伴非快速眼动睡眠期睡眠障碍,104例)。所有患者均接受多导睡眠图监测,对比两组总睡眠时间(TST)、睡眠潜伏期(SOL)、觉醒次数(WUT)、睡眠效率(SE)、快速眼动睡眠(REM)期睡眠时间(REMT)、REM期睡眠占总睡眠时间百分比(REM%)及非快速眼动期(NREM)睡眠S1期、S2期、S3+S4期睡眠时间。采用蒙特利尔认知评估量表(MOCA)评估两组患者认知功能,并分析两组血清S-100β水平及其与上述睡眠参数和认知功能的关系。结果:A组TST、SE、REMT、REM%及NREM睡眠S2、S3+S4期睡眠时间显著低于B组(P<0.05),SOL、WUT及NREM睡眠S1期睡眠时间显著高于B组(P<0.05)。A组视空间与执行功能、注意、延迟记忆、定向力评分及MOCA总分均显著低于B组(P<0.05),命名、语言、抽象评分较B组比较差异无统计学意义(P>0.05)。A组血清S-100β水平显著低于B组(P<0.05)。老年急性脑梗死伴非快速眼动期睡眠障碍患者血清S-100β与TST、SE、REMT、REM%、S2、S3+S4呈负相关(r=-0.466、-0.682、-0.612、-0.646,P均<0.05),与SOL、WUT、S1呈正相关(r=0.435、0.587、0.187,P均<0.05),与MOCA总分呈负相关(r=-0.327,P<0.05)。结论:血清S-100β与老年急性脑梗死伴非快速眼动期睡眠障碍患者睡眠质量及认知功能相关。Objective:To study the relationship between serum S-100βand sleep disorder and cognitive function sleep in elderly patients with acute cerebral infarction.Methods:216 elderly patients with acute cerebral infarction admitted to our hospital from May 2015 to May 2019 were divided into group A(with non-rapid eye movement sleep disorder,112 cases)and group B(without non-rapid eye movement sleep disorder,104 cases).All patients accept guide more sleep monitoring,compared two groups of total sleep time(TST),sleep latency(SOL),number of awakening(WUT),sleep efficiency(SE)and rapid eye movement(REM)sleep period(REMT),REM sleep time period of total sleep time percentage(REM%)sleep and non-rem sleep(NREM)sleep stage S1,S2,S3+S4 sleep time.Montreal Cognitive Assessment Scale(MOCA)was used to assess the cognitive function of the two groups.Serum S-100beta level and its relationship with sleep parameters and cognitive function were analyzed.Results:The sleep time of TST,SE,REMT,REM%and NREM in group A was significantly lower than that in group B(P<0.05),while the sleep time of SOL,WUT and NREM in stage S1 was significantly higher than that in group B(P<0.05).The scores of visual space and executive function,attention,delayed memory,orientation and MOCA in group A were significantly lower than those in group B(P<0.05),but there was no significant difference in naming,language and abstract scores between group A and group B(P>0.05).Serum S-100beta level in group A was significantly lower than that in group B(P<0.05).In elderly patients with acute cerebral infarction with non-rapid eye movement sleep disorder,serum s-100 was negatively correlated with TST,SE,REMT,REM%,S2,S3+S4(r=-0.466,-0.682,-0.612,-0.646,all P<0.05),positively correlated with SOL,WUT,and S1(r=0.435,0.587,0.187,all P<0.05),and negatively correlated with the total score of MOCA(r=-0.327,P<0.05).Conclusion:Serum S-100beta is associated with sleep quality and cognitive function in elderly patients with acute cerebral infarction and non-REM sleep disorder.
