摘  要:目的探讨2型糖尿病(T2DM)合并糖尿病周围神经病变(DPN)患者血清神经胶质纤维酸性蛋白(GFAP)水平及其临床意义。方法将100例T2DM患者根据有无DPN分为T2DM组和DPN组,每组50例,并另选取50例健康体检者为对照组。比较3组研究对象的空腹血糖、糖化血红蛋白A1c(HbA1c)、GFAP水平及神经传导速度,并分析DPN患者GFAP水平与空腹血糖、HbA1c及神经传导速度的相关性。结果对照组的空腹血糖、HbA1c水平均低于T2DM组和DPN组,对照组、T2DM组、DPN组GFAP水平依次升高(均P<0.05)。DPN组的正中神经运动神经传导速度(MMCV)、腓总神经运动神经传导速度(PMCV)、正中神经感觉神经传导速度(MSCV)和腓总神经感觉神经传导速度(PSCV)均慢于对照组,MMCV、PMCV均慢于T2DM组(均P<0.05)。DPN组患者血清GFAP水平与糖尿病病程呈正相关,与MMCV、PMCV、MSCV和PSCV呈负相关(均P<0.05)。结论 T2DM合并DPN患者GFAP水平升高,其可反映T2DM患者神经系统损伤情况,有助于DPN的诊断。Objective To investigate serum glial fibrillary acidic protein(GFAP) level and its clinical significance in patients with type 2 diabetes mellitus(T2 DM) complicated with diabetic peripheral neuropathy(DPN). Methods A total of 100 patients with T2 DM were divided into T2 DM group and DPN group according to the presence of DPN, with 50 cases in each group, and another 50 healthy check-up individuals were selected as control group. The levels of fasting blood glucose, hemoglobin A1 c(HbA1 c) and GFAP as well as nerve conduction velocity were compared among the three groups, and the correlation of GFAP level with fasting blood glucose, HbA1 c or nerve conduction velocity was analyzed in DPN patients. Results The control group possessed lower levels of fasting blood glucose and HbA1 c than the T2 DM and DPN groups, and the levels of GFAP in the control group, the T2 DM group and the DPN group increased in turn(all P<0.05). The DPN group exhibited slower motor nerve conduction velocity of median nerve(MMCV), motor nerve conduction velocity of peroneal nerve(PMCV), sensory nerve conduction velocity of median nerve(MSCV), and sensory nerve conduction velocity of peroneal nerve(PSCV) in comparison with the control group, as well as slower MMCV and PMCV than the T2 DM group(all P<0.05). In the DPN group, serum GFAP level positively correlated with the course of diabetes, and negatively correlated with MMCV, PMCV, MSCV and PSCV(all P<0.05). Conclusion The level of GFAP is elevated in T2 DM patients complicated with DPN, which represents nerve system impairment in T2 DM patients and might contribute to DPN diagnosis.
