摘 要:目的探讨谷胱甘肽硫转移酶M1(glutathione S-transferase M1,GSTM1)基因多态性与中国人群头颈恶性肿瘤易感性的关系。方法计算机检索Web of Science、Pubmed、Embase、CNKI、VIP和万方等中英文数据库,检索时限从建库截至2018年12月,由两名耳鼻咽喉头颈外科专业研究生按照纳入排除标准严格筛选文献并提取资料,采用NOS量表评价纳入研究文献的质量,应用Stata 12.0软件进行Meta分析,采用优势比(odds ratio,OR)及95%可信区间(95%confidence interval,95%CI)描述关联强度,并按照肿瘤部位、地区及对照来源进行亚组分析。结果纳入文献21篇,其中包含3735例病例组和4995对照组,Meta分析结果显示:GSTM1无效基因型与中国人群头颈恶性肿瘤发病风险增加具有相关性(OR=1.78,95%CI:1.47-2.16);亚组分析表明,GSTM1无效基因型可能增加中国大陆人群头颈恶性肿瘤的发病风险(OR=2.02,95%CI:1.68-2.43),尤其是鼻咽癌(OR=1.53,95%CI=1.32-1.78)和喉癌(OR=2.56,95%CI=1.82-3.59),且在医院为基础的病例对照研究(OR=2.48,95%CI=1.56-3.96)和社区为基础的病例对照研究(OR=1.68,95%CI=1.32-2.15)中均有统计学意义。结论在中国人群中GSTM1基因多态性与头颈恶性肿瘤易感性相关联。OBJECTIVE To investigate the relationship between glutathione S-transferase M1(GSTM1)gene polymorphism and susceptibilit y to head and neck cancer in Chinese populat ion.METHODS Computers searched Chinese and English databases such as Web of science,Pubmed,Embase,CNKI,VIP and Wanfang.The search time limit was from the establishment of the library to December 2018.Two graduate students of Otolaryngology Head and Neck Surgery strictly selected the literature according to the inclusion exclusion criteria.Data were extracted,and the quality of the included literature was evaluated by NOS scale.Finally,Meta analysis was performed using Stata 12.0 software,using odds ratio(OR)and 95%confidence interval(95%CI).The association intensity is described and subgroup analysis is performed according to the location of the head and neck cancer,the region,and the control source.RESULTS A total of 21 articles were included in the literature,including 3735 cases and 4995 controls.Meta-analysis showed that GSTM1 null genotype was associated with increased risk of head and neck cancer in Chinese population(OR=1.78,95%CI:1.47-2.16);Subgroup analysis showed that GSTM1 null genotype may increase the risk of head and neck cancer in the Chinese mainland population(OR=2.02,95%CI:1.68-2.43),especially nasopharyngeal carcinoma(OR=1.53,95%CI:1.32-1.78)and laryngeal cancer(OR=2.56,95%CI:1.82-3.59),and a hospital-based case-control study(OR=2.48,95%CI:1.56-3.96)and A community-based case-control study(OR=1.68,95%CI:1.32-2.15)was statistically significant.CONCLUSION In the Chinese population,GSTM1 gene polymorphism is associated with head and neck cancer susceptibility.
