甲型流感病毒的感染与白细胞VCS参数的相关性分析

摘　　要：目的探讨甲型流感患者静脉血白细胞体积-电导率-激光散射率(VCS)参数的变化规律,白细胞VCS参数在甲型流感病毒感染中的临床诊断价值.方法收集甲型流感病毒感染患者101例(甲流组),同期体检健康者101例(对照组).利用BeckmanCoulterDxH800全自动血细胞分析仪对甲流组和对照组进行静脉血白细胞VCS参数测定及统计学分析.结果甲型流感患者中心粒细胞平均低中位角光散射(MN-LMALS-NE)与对照组相比,差异无统计学意义(P>0.05);与对照组相比较,中性粒细胞平均电导率(MN-C-NE)、中心粒细胞平均低角度光散射(MN-LALS-NE)、淋巴细胞平均电导率(MN-C-LY)和单核细胞平均低角度光散射角度光散(MN-LALS-MO)呈显著性降低(P<0.05);其余参数呈显著性升高(P<0.05).ROC曲线显示白细胞VCS参数在甲型流感的诊断中具有很好的灵敏度和特异性,其中淋巴细胞和单核细胞的平均中位角光散射(MN-MALS)、平均高中位角光散射(MN-UMALS)和平均低中位角光散射(MN-LMALS)的曲线下面积(AUC)>0.99,灵敏度和特异性均>90%.结论白细胞VCS参数能较好反映甲型流感病毒感染状态白细胞形态、结构的变化,对辅助诊断甲型流感具有很高的潜在价值.Objective To study the changes of volume-conductiviry-scatter rate(VCS)parameters of leukocytes in patients with influenza A,and to explore the clinical diagnostic value of VCS parameters of leukocytes in influenza A vims infection.Methods 101 patients with influenza A vims infection and 101 healthy people were collected,and were respectively set as a infection group and a control group.The VCS parameters of leukocytes were detennined by a Beckman Coulter DxH800 in the infection group and the control group,and the results were analyzed.Results There was no significant difference in MN-LMALS-NE between the infection group and the control group(P>0.05).Compared with the control group,MN-C-NE,MN-LALS-NE,MN-C-LY and MN-LALS-MO showed a significant decrease(P<0.05).The remaining parameters were significantly increased(P<0.05).The ROC curve showed that the VCS parameters had good sensitivity and specificity in the diagnosis of influenza A,and the area under the curve(AUC)of MN-MAL,MN-UMALS and MN-LMALS of lymphocytes and monocytes was as high as 0.99 and Above,the sensitivity and specificity were both above 90%.Conclusion The VCS parameters can well reflect the changes in the morphology and structure ofleukocytes in the influenza A vims infection state,and have high potential value for the auxiliary diagnosis of influenza A.
