肌电图检查在上肢周围神经卡压征与双卡综合征诊断中的价值

摘　　要：目的探讨肌电图(EMG)检查在上肢周围神经卡压征(PNC)与双卡综合征(DCS)中的诊断价值.方法2017年1月至2019年1月265例疑诊为上肢PNC患者进行EMG检查,常规检测正中、尺、桡、肌皮、腋神经、前臂内侧皮神经、前臂外侧皮神经以及正中、尺神经的F波,两侧对比并分析检查结果.结果265例患者中,符合单侧腕管综合征(CTS)EMG表现70例(26.41%),双侧CTS46例(17.36%),单侧肘管综合征(CubTS)27例(10.19%),双侧CubTS16例(6.04%),胸廓出口综合征(TOS)1例(0.38%),旋前圆肌综合征1例(0.38%),神经根型颈椎病17例(6.42%),CTS合并CubTS6例(2.26%),腕管综合征合并神经根型颈椎病21例(7.92%),肘管综合征合并神经根型颈椎病3例(1.13%),正常57例(21.50%).结论神经肌电图检测在上肢周围神经卡压与双卡综合征的诊断中有非常重要的意义,临床可根据肌电图检查结果指导治疗.Objective To investigate the significance of EMG in the diagnosis of PNC and DCS.Methods From January 2017 to January 2019,265 outpatients suspected of upper limb peripheral nerve compression were examined by EMG.The F-wave of median,ulnar,radial,musculocutaneous,axillary nerve,medial foreann cutaneous nerve,lateral forearm cutaneous nerve and median and ulnar nerve were detected conventionally,the two sides were compared and the results were an;dyzed.Results Among the 265 patients suspected of upper limb periphenil nerve entrapment of(PNC),there were 70 cases(26.41%)of unilateral carpal tunnel syndrome(CTS),46 cases(17.36%)of bilateral CTS,27 cases(10.19%)of unilateral CubTS,16 cases(6.04%)of bilateral CubTS,1 case(0.38%)of TOS,1 case(0.38%)of teres pronator syndrome,17 cases(6.42%)of CSR,6 cases(2.26%)of CTS with CubTS,21 cases(7.92%)of CTS with CSR,3 cases(1.13%)of CubTS with CSR,and 57 cases(21.50%)were normal.Conclusion EMG plays an important role in the diagnosis of PNC and DCS,and the clinical treatment can be guided by EMG results.
