超声内镜引导细针穿刺活检对胰腺占位性疾病的诊断价值

摘　　要：目的评价超声内镜引导细针穿刺活检(endoscopic ultrasonography-guided fine needle aspiration,EUS-FNA)对胰腺占位性疾病的诊断价值。方法收集西安交通大学第一附属医院2013年6月至2018年7月影像学(CT/MRI/经腹B超)提示胰腺占位性疾病的患者,行EUS-FNA检查,观察EUS-FNA穿刺情况与病理报告结果,分析诊断灵敏度与特异度,并将超声内镜(endoscopic ultrasonography,EUS)与影像学检查的诊断结果相关指标进行比较。结果患者共96例,男性69例,女性27例,平均年龄(60.5±11.9)岁。EUS-FNA诊断的灵敏度为91.9%,特异度为100.0%,准确率为93.8%。EUS对胰腺占位性疾病诊断的灵敏度、特异度及准确率均高于CT、MRI及经腹B超。所有患者仅有1例穿刺点少量出血,给予200 mL/L孟氏液20 mL喷洒后无出血,其余均未出现并发症。结论EUS-FNA是一项灵敏度、准确性、安全性均较高的诊断技术,有助于早期诊断胰腺肿瘤。Objective To evaluate the diagnostic value of endoscopic ultrasonography-guided fine needle aspiration(EUS-FNA)for pancreatic space-occupying diseases.Methods We recruited EUS-FNA patients from The First Affiliated Hospital of Xi an Jiaotong University from June 2013 to July 2018,who had images(CT,MRI or transabdominal B-ultrasound)suggesting pancreatic masses.We observed the puncture of EUS-FNA and the pathological results,analyzed the sensitivity and specificity of EUS-FNA diagnoses,and compared the results of EUS with the images.Results There were 96 patients(69 males and 27 females),with an average age of(60.5±11.9)years.The sensitivity of EUS-FNA was 91.9%,the specificity was 100.0%,and the accuracy was 93.8%.The sensitivity,specificity and accuracy of EUS were higher than those of CT,MRI and transabdominal B-ultrasound.Only one patient had little bleeding at the puncture site,and there was no bleeding with spraying 20%of Monsel s solution.The other patients had no complications.Conclusion EUS-FNA is a diagnostic technique with high sensitivity,accuracy and safety,which can help detect pancreatic tumors in the early stage.
