摘  要:目的观察卒中后认知障碍患者血清中miR-191-5p和miR-20a-5p变化,以探讨两者联合是否能够作为脑卒中后认知障碍的诊断标志物。方法收集36例脑卒中后认知障碍(post-stroke cognitive impairment,PSCI)患者、36例脑卒中后认知正常(post-stroke cognitive normal,PSCN)患者和37例年龄匹配对照组(age-matched control,AMC)的样本。使用ROC曲线和AUC分析比较miR-191-5p和miR-20a-5p及其联合对PSCI的诊断能力。结果与PSCN和AMC相比,PSCI患者血清中,miR-191-5p和miR-20a-5p均升高,ROC曲线显示miR-191-5p的AUC(95%CI)为0.647(P<0.05),灵敏度为66.7%,特异度为55.6%;miR-20a-5p的AUC(95%CI)为0.759(P<0.05),灵敏度为77.8%,特异度为61.1%;两者联合的AUC(95%CI)为0.861(P<0.05),灵敏度为88.9%,特异度为72.2%。而且miR-191-5p和miR-20a-5p均与PSCI患者的MoCA评分相关,miR-191-5p(r=-0.488,P<0.01),miR-20a-5p(r=-0.624,P<0.01)。结论血清miR-191-5p和miR-20a-5p表达与卒中后认知功能存在显著相关性,提示miR-191-5p联合miR-20a-5p可能是PSCI的诊断标志物。Objective To observe the changes of serum miR-191-5 p and miR-20 a-5 p in patients with poststroke cognitive impairment(PSCI)and study whether the combination of them can be used as a diagnostic marker of PSCI.Methods Collect 36 PSCI patients,36 post-stroke cognitive normal(PSCN)patients,and 37 age-matched controls(AMC)were enrolled.ROC curve and AUC analysis were used to compare the diagnostic ability of miR-191-5 p,miR-20 a-5 p and their combination in PSCI.Results Compared with PSCN and AMC,miR-191-5 p and miR-20 a-5 p in serum of patients with PSCI were higher.The ROC curve showed that the AUC(95%CI)of miR-191-5 p was 0.647(P<0.05),the sensitive was 66.7%,and the specificity was 55.6%.The AUC(95%CI)of miR-20 a-5 p was 0.759(P<0.05),the sensitive was 77.8%,and the specificity was 61.1%.The combined AUC(95%CI)was0.861(P<0.05),the sensitive was 88.9%,and the specificity was 72.2%.Both miR-191-5 p and miR-20 a-5 p were correlated with MoCA score of PSCI patients,miR-191-5 p(r=-0.488,P<0.01),miR-20 a-5 p(r=-0.624,P<0.01).Conclusion The expression of serum miR-191-5 p and miR-20 a-5 p are significantly correlated with cognitive function after stroke,suggesting that miR-191-5 p combined with miR-20 a-5 p may be a diagnostic marker of PSCI.
