摘  要:目的探讨胃癌组织中HOXC10表达水平与患者临床病理特征及预后的关系。方法采用荧光定量PCR和免疫组织化学法分别检测20对胃癌组织及相应癌旁组织和264例患者胃癌病理切片中HOXC10的表达水平,采用χ^2检验分析胃癌组织中HOXC10的表达水平与患者临床病理特征的关系。采用单因素、多因素Cox回归分析评估HOXC10表达水平与无复发生存期(RFS)、总生存期(OS)的关系。用二元Logistic回归分析建立预测患者5年内复发或死亡风险的模型。结果与HOXC10高表达组相比,HOXC10低表达组的RFS和OS更长(P<0.05),病理分化程度更好(P=0.001),淋巴结转移更少(P=0.001),TNM分期更早(P=0.001)。单因素Cox回归分析显示,病理分级、Lauren分型、TNM分期、T分期、N分期及HOXC10表达水平均与RFS及OS显著相关(P<0.05);多因素Cox回归分析显示,病理分级、Lauren分型、TNM分期、T分期、N分期及HOXC10表达水平均与RFS及OS显著相关(P<0.05)。基于传统临床病理特征参数的Logistic回归模型与基于HOXC10参数的Logistic回归模型对胃癌患者术后5年内复发或死亡风险的预测效果差异无统计学意义(P>0.05)。结论HOXC10低表达的胃癌患者的Lauren分型以肠型为主,分化程度较好,TNM分期较早,淋巴结转移较少,预后较好。HOXC10可作为胃癌患者预后的预测指标。在淋巴结转移情况不明时,以HOXC10为参数的Logistic回归模型可辅助预测预后。Objective This paper intends to investigate the relationship between the expression of HOXC10 in gastric carcinoma tissue and the clinicopathologic characteristics and prognosis of patients.Methods Using fluorescence quantitative PCR and immunohistochemistry,the expression of HOXC10 in the pathological sections of 20 pairs of gastric carcinoma and the corresponding adjacent tissues and 264 patients with gastric carcinoma was detected.The relationship between the expression of HOXC10 in the gastric cancer tissue and the clinicopathological characters was analyzed by means of chi-square test.The single factor and multi-factor Cox regression analysis were used to analyze the relationship between the HOXC10 expression and relapse free survival(RFS)and overall survival(OS).Using binary Logistic regression analysis,a model was established to predict the risk of recurrence or death within 5 years.Results Both RFS and OS in the HOXC10 low expression group were superior to those in the high expression group(P<0.05),with a better differentiation(P=0.001),less lymph node metastasis(P=0.001),and earlier TNM staging(P=0.001).The single factor Cox regression analysis showed that pathological grades,Lauren classification,TNM staging,T staging,N staging,and HOXC10 expression were significantly correlated with RFS and OS in patients with gastric cancer(P<0.05).The multiple factor Cox regression analysis showed that pathological grades,Lauren classification,TNM staging,T staging,N staging,and HOXC10 expression were significantly correlated with RFS and OS in patients with gastric cancer(P<0.05).Compared with the Logistic regression model containing only traditional clinicopathological parameters,the Logistic regression model containing HOXC10 as a parameter had no significant difference in predicting the risk of recurrence or death within 5 years after operation in patients with gastric cancer(P>0.05).Conclusions Gastric cancer patients with low-expression OXC10 are more common in intestinal type,with a good degree of differentiat
