摘　　要：目的探究CTGF mRNA在溃疡性结肠炎(UC)组织中的表达水平及与病情和预后的关系。方法选择2014年2月至2017年4月在广元市第一人民医院就诊的83例UC患者作为研究对象(设为UC组),另择同期经结肠镜检查并切除结肠息肉的85例患者作为对照组。采用RT-qPCR法检测两组患者结肠组织中CTGF mRNA的相对表达量,并分析其与UC组患者的病情和预后的关系。结果UC组患者结肠组织中CTGF mRNA的相对表达量高于对照组,差异有统计学意义(P<0.05)。重度活动度UC患者结肠组织中CTGF mRNA的相对表达量高于轻、中度活动度UC患者,差异有统计学意义(P<0.05)。UC患者结肠组织中CTGF mRNA的相对表达量与UCEIS评分呈正相关(P<0.05)。在83例UC患者中有50例(60.24%)预后良好,33例(39.76%)预后不良。CTGF mRNA诊断UC患者预后不良的受试者工作特征(ROC)曲线下面积(AUC)为0.925,最大约登指数为0.699,cut-off值为1.86,敏感度为93.94%,特异度为76.00%。多因素Logistic回归分析显示UCEIS评分和CTGF mRNA相对表达量均与UC患者的预后密切相关(P<0.05)。结论CTGF mRNA在UC组织中高表达,检测其相对表达量有助于评估UC患者的病情及预后。Objective This paper is to investigate the expression of CTGF mRNA in ulcerative colitis(UC)and its relationship with disease and prognosis.Methods Eighty-three UC patients who were treated in Guangyuan First People′s Hospital,Guangyuan,Sichuan from February 2014 to April 2017 were selected and assigned to the UC group,and 85 patients who underwent colonoscopy and resected colon polyps were assigned to the control group.RT-qPCR was used to detect the relative expression of CTGF mRNA in colon tissue and analyze its relationship with the condition and prognosis of UC patients.Results The relative expression of CTGF mRNA in the colon tissue of patients in the UC group was higher than that in the control group(P<0.05).The relative expression of CTGF mRNA in colon tissue of the patients with severe activity was higher than that of the patients with mild to moderate mobility(P<0.05).The relative expression of CTGF mRNA in the colon tissue of the patients in the UC group was positively correlated with the UCEIS score(P<0.05).Of the 83 patients in the UC group,fifty(60.24%)had a good prognosis,and 33(39.76%)had a poor prognosis.The AUC of CTGF mRNA in the diagnosis of the patients in the UC group with poor prognosis was 0.925,and the most approximate index,cut-off value,sensitivity,and specificity were 0.699,1.86,93.94%,and 76.00%,respectively.The multivariate logistic regression analysis showed that the UCEIS score and the CTGF mRNA relative expression were closely related to the prognosis of the patients in the UC group(P<0.05).Conclusions CTGF mRNA is highly expressed in the UC group.The relative expression of CTGF mRNA is helpful to evaluate the condition and prognosis of patients in the UC group.
