癫痫与偏头痛患者临床相关特点分析

摘　　要：目的:探讨癫痫、偏头痛患者共患病及脑电图情况、疗效等相关临床特点。方法:回顾性分析2017年1月至2017年12月神经内科或门诊就诊的偏头痛患者、癫痫患者、癫痫共患偏头痛患者各25例共75例,接受相同的脑电图检查,分析检查结果及根据相关指南治疗并进行疗效评价。结果:脑电图结果异常率在癫痫组为80%,偏头痛组为48%,癫痫共患偏头痛组为84%,3组数据比较,差异有统计学意义(P<0.05);经规范抗癫痫治疗单纯癫痫组患者治疗有效率为88%,癫痫共患偏头痛患者有效率为68%,单纯癫痫组治疗有效率高于癫痫共患偏头痛组(P<0.05);按中国偏头痛指南规范治疗单纯偏头痛患者头痛治疗有效率为84%,癫痫共患偏头痛患者治疗有效率为56%,癫痫共患偏头痛组患者治疗有效率低于单纯偏头痛组(P<0.05)。结论:脑电图检查用于偏头痛、癫痫和癫痫共患偏头痛诊断中有一定临床价值,癫痫共患偏头痛患者癫痫控制及头痛控制有效率较差,有一定临床指导价值。Objective:To investigate the clinical characteristics of EEG and curative effect of patients with epilepsy and migraine.Methods:75 patients with migraine,epilepsy and epilepsy with migraine from January 2017 to December 2017 who were randomly admitted to the neurology department or outpatient department in our hospital were analyzed retrospectively.They received the same general electroencephalogram examination.The results were analyzed and the therapeutic effect was evaluated according to the relevant guidelines.Results:The abnormal rate of EEG was 80%in the epilepsy group,48%in the migraine group and 84%in the epilepsy and migraine group.There was statistical difference between the three groups(P<0.05).The effective rate of standard antiepileptic therapy in simple epilepsy group was 88%,and that of epilepsy patients with migraine was 68%.There was statistical difference between the two groups(P<0.05).According to the Chinese migraine guidelines,the effective rate of headache treatment for simple migraine patients was 84%,and that for epilepsy patients with migraine was 56%.There was significant difference between the two groups(P<0.05).Conclusion:EEG has certain clinical value in the diagnosis of migraine,epilepsy and epilepsy with migraine.The effective rate of epilepsy control and headache control is poor in patients with epilepsy and migraine,which is of a certain clinical value.
