摘　　要：目的对比三种不同器械用于磨牙弯曲根管牙髓摘除的效率。方法按Schneider根管弯曲度测量法[1],从2017年7月~2019年3月在湖北省第三人民医院口腔科就诊的20~60岁磨牙急性牙髓炎患者中选择弯曲根管(≧20°)的患牙128颗,随机分三组,A组采用拔髓针结合不锈钢K挫预备后封Ca(OH)2糊剂;B组采用Wave one预备后封Ca(OH)2糊剂;C组采用M3-L预备后封Ca(OH)2糊剂。记录三组根管预备的时间和治疗后复诊时根管内探痛的发生率。结果A组平均根管预备时间为(14.75±2.17)min,复诊时根管内探痛发生率为29.6%;B组平均根管预备时间为(10.12±1.6)6min,复诊时根管内探痛发生率为13.3%;C组平均根管预备时间(6.01±0.84)min,复诊时根管内探痛发生率为2.7%。各组间根管预备平均时间的差异有统计学意义(P<0.05);各组间根管探痛率的差异也都有统计学意义(P<0.05)其中A组根管预备的平均时间最长,根管探痛率最高,C组根管预备的平均时间最短,根管探痛率最低。结论对于磨牙弯曲根管,M3-L由于其良好的柔韧性,可记忆性能,能快速,高效地清理牙髓组织,值得临床推广。Objective To evaluate the cleaning effects of three kinds of pulpectomy on molar curved root canals.Methods A total of 128 molars with acute pulpitis and curved root canals(≧20°)which treated in The Third people’s Hospital of Hubei province from July 2017 to March 2019 were selected and randomly divided into three groups.The pulp was removed by nerve broach and the root canals were prepared with stainless steel K-file,then covered with calcium hydroxide in group A;The preparation was carried out with Wave One instruments in the group B and M3-L NiTi rotary instruments in the group C.The average root canal preparation time and the rate of pain in canal at subsequent visit were recorded.Results The average root canal preparation time was(14.75±2.17)min,and the rate of pain occurrence at subsequent visit was 29.6%in group A;The average root canal preparation time was(10.12±1.66)min,and the rate of pain occurrence at subsequent visit was 13.3%in group B;The average root canal preparation time was(6.01±0.84)min,and the rate of pain occurrence at subsequent visit was 2.7%in group C.The mean time of root canal preparation in each group was statistically significant(P<0.05).The difference rate of root canal pain between each group was also statistically significant(P<0.05).Among them,the average preparation time of group A was the longest and the rate of root canal pain was the highest,while the average preparation time of group C was the shortest and the rate of root canal pain was the lowest.Conclusion For the curved root canal of molars,M3-L is worth promoting on clinical due to its good flexibility,memory behaviour and can clean the pulp tissues quickly and efficiently.
