摘　　要：我国直肠癌以中低位为主。随着对低位直肠解剖、生理功能及直肠癌肿瘤生物学行为的不断认识,括约肌间切除术(in?tersphincteric resection,ISR)成为极限保肛术式,使部分低位直肠癌患者在保证肿瘤学疗效的基础上免遭切除肛门的痛苦。随着腹腔镜技术的快速发展,腹腔镜经腹ISR手术在保证肿瘤根治基础上,在减少术中并发症、提高手术安全性、保护器官神经功能及改善术后排便功能方面存在一定优势。Mid and low rectal cancer are the most common types of rectal cancer in China.With advances in understanding the anatomy,physiological function of low rectal,and biological behavior of low rectal cancer,intersphincteric resection(ISR)emerges as the ultimate anal preservation surgery which spares anus resection while achieving satisfactory oncologic effect.With the rapid development of laparoscopy,laparoscopic transabdominal ISR has advantages in terms of reducing intraoperative compli?cations,improving surgical safety,protecting nerve function,and improving postoperative defecation function,while ensuring radi?cal resection of the tumor.
