摘　　要：目的探讨椎管内畸胎瘤的MRI及临床表现.方法对37例经手术病理证实的椎管内畸胎瘤患者的临床及MRI影像资料进行回顾性分析.结果37例病灶(颈段1例,胸段6例,腰段30例)中位于髓外硬膜下23例,脊髓内14例;病变MRI表现不一,24例病灶同时含脂肪组织及钙化,信号混杂,13例为囊实性占位;35例患者行MRI增强检查,14例未强化,21例明显不均匀强化;6例患者伴脊髓栓系,2例同时伴有脊柱纵裂.患者临床表现不一,多为疼痛、感觉异常,反射异常等,与病灶定位及大小相关;另有1例患者无明显临床症状.结论椎管内畸胎瘤MRI特征性表现为各序列混杂信号.结合MRI表现及临床症状可提高术前确诊率,并指导临床治疗.Objective To investigate the MRI features and clinical manifestations of intraspinal teratoma.Methods The clinical manifestations and MRI findings of 37 cases with intraspinal teratoma confirmed by operation and pathology were analyzed retrospectively.Results Of the 37 lesions(1 in the cervical segment,6 in the thoracic segment and 30 in the lumbar segment),23 lesions were extramedullary subdural,14 lesions were intramedullary.MRI characteristic of 24 lesions that contained both adipose tissue and calcification were heterogeneous signal on all sequences,the other 13 cases mainly presented as solid-cystic lesions.35 patients underwent MRI-enhanced examination,14 cases were unenhanced,and 21 cases were significantly unevenly enhanced.6 cases were with tethered spinal cord,in which 2 cases were accompanied by split cord malformation.The multiple clinical manifestations were pain,paresthesia,abnormal reflex,and etc,which were related to the location and size of the lesion.There was 1 patient who had no obvious clinical symptoms.Conclusion MRI can demonstrate intraspinal teratoma well with characteristic heterogeneous signal on all sequences.Combined with MRI findings and clinical symptoms,the preoperative diagnosis rate can be improved,which can be used to guide clinical treatment.
