舒洛地特预防老年恶性肿瘤化疗患者下肢深静脉血栓效果观察

摘　　要：目的观察舒洛地特预防老年恶性肿瘤化疗患者下肢深静脉血栓的临床效果。方法选取2017年1月-2018年12月南通瑞慈医院收治的老年恶性肿瘤化疗患者88例,采用随机数字表法分为观察组及对照组各44例。对照组化疗开始时实施健康教育、饮食干预、床上运动、保证水电解质平衡等,观察组化疗开始时给予舒洛地特。比较2组患者治疗前后血清学指标,包括D-二聚体浓度、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT),比较2组下肢深静脉血栓及不良反应发生情况。结果观察组D-二聚体水平低于对照组,PT、APTT均长于对照组(P均<0. 01)。观察组下肢深静脉血栓发生率为0,低于对照组的9. 09%(4/44)(χ^2=4. 04,P <0. 05)。2组均未出现消化道出血、牙龈出血及皮肤出血等不良反应。结论舒洛地特可有效降低老年恶性肿瘤化疗患者下肢深静脉血栓的发生率,效果较好,值得临床推广应用。Objective To investigate the value of sulodexide in preventing deep venous thrombosis of lower limbs in elderly patients with malignant tumors undergoing chemotherapy. Methods From January 2017 to December 2018,88 elderly patients with malignant tumors undergoing chemotherapy were admitted to Nantong Rich Hospital. Random number table method was used to divide the observation group and control group into 44 cases each. The control group was given health education,dietary intervention,bed exercise,water and electrolyte balance,etc. The observation group received sulodexter at the beginning of chemotherapy. Compared the serum parameters before and after treatment in the two groups,including D-dimer concentration,prothrombin time( PT),activated partial thromboplastin time( APTT). The incidence of deep vein thrombosis and adverse reactions in the lower limbs of the two groups were compared. Results The D-dimer level in the observation group was lower than that in the control group,and both PT and APTT were longer than those in the control group( P < 0. 01).The incidence of deep vein thrombosis in the lower limbs of the observation group was 0,which was lower than 9. 09%( 4/44)in the control group( χ^2= 4. 04,P < 0. 05). There were no adverse reactions such as gastrointestinal bleeding,gum bleeding,and skin bleeding in both groups. Conclusion Sulodipine can reduce the incidence of deep venous thrombosis of lower limbs in elderly patients with malignant tumors undergoing chemotherapy,which has good effect and is worthy of clinical application.
