摘  要:目的研究炎性因子在精神分裂症患者并发糖尿病过程中所起作用,分析精分人群高发糖尿病的原因,检测促炎因子和抑炎因子的表达水平,在精神分裂症及其并发症之间所扮演角色,探讨其临床意义,以期明确精神分裂症患者与炎症因子的关系,延缓糖尿病的发生,为规范用药和抗炎治疗提供临床依据。方法本研究选定三组人群,健康对照组(NC组)、精神分裂症患者(SP组),精神分裂症合并糖尿病患者(SPDM组),检测3组患者血液中单个核细胞炎性因子IL-1β、TNF-α和IL-10mRNA表达水平,并分析其与三酰甘油,总胆固醇,血糖和BMI的相关性。结果SP组与SPDM组的BMI水平相比NC组显著高(P<0.0001)。Real-time PCR检测结果显示:IL-10mRNA相对表达值,SP组(2.64±1.59)IL-10mRNA与正常对照组NC组(1.05±0.58)相比,差异有统计学意义(P<0.05);IL-1βmRNA的相对表达值,SP组(13.5±7.53),SPDM组(29.18±10.96)与正常对照组NC组(1.21±0.81)相比,差异有统计学意义(P<0.05);TNF-α的相对表达值NC组(1.63±0.55),SP组(4.49±2.37),SPDM组(11.04±5.78)与NC组相比差异有统计学意义(P<0.05)。相关分析:在SP组,IL-1βmRNA的表达水平和BMI呈负相关关系(r=-0.3965,P=0.041);在SP+SPDM组中,IL-1βmRNA的表达水平和FPG呈正相关关系(r=0.3562,P=0.3562)。结论炎性因子可能是精神分裂症患者发病的重要相关原因之一,同时与精神分裂症患者罹患糖尿病也有关联。而炎性因子的升高与精神分裂症患者BMI的改变有关。同时提示对炎性因子的研究不能局限于一种,应当建立炎性因子组合。Objective To investigate the role of inflammatory factors in the pathogenesis of schizophrenia complicated by diabetes mellitus,to analyze the causes of high incidence of diabetes in those with schizophrenia,to detect the expression levels of pro-inflammatory and anti-inflammatory factors,their role in schizophrenia and its complications and their clinical significance,in order to determine the relationship between schizophrenia and inflammatory factors,delay the occurrence of diabetes,and provide a clinical basis for standardized medication and anti-inflammatory treatment.Methods Three groups of patients were enrolled in the study,including healthy control group(NC group),schizophrenia group(SP group),schizophrenia with diabetes mellitus group(SPDM group).The expression levels of blood mononuclear inflammatory factor IL 1β,TNF-αand IL-10 mRNA were detected in the three groups and their correlation with triacylglycerol,total cholesterol,blood glucose and BMI was analyzed.Results The BMI levels in SP group(25.88±3.18)and the SPDM group(25.97±3.13)were significantly higher than those(22.14±2.68)in NC group(P<0.01).The results of Real-time PCR showed that there were significant differences in the relative expression levels of IL-10 mRNA between the SP group(2.64±1.59)and the NC group(1.05±0.58,P<0.05).The relative expression levels of IL-1βmRNA in SP group(13.5±7.53)and SPDM group(29.18±10.96)were significantly different from those(1.21±0.81)in NC group(P<0.05).There were significant differences in the relative expression levels of TNF-αbetween NC group(1.63±0.55)and SP group(4.49±2.37),SPDM group(11.04±5.78,P<0.05).Correlation analysis showed that IL-1βmRNA expression levels were negatively correlated with BMI levels in SP group(r=-0.3965,P=0.041),however,which in SP+SPDM group were positively correlated with FPG levels(r=0.3562,P=0.3562).Conclusion Inflammatory factors may be one of the important related causes of schizophrenia,which is also associated with diabetes in patients with schizophrenia.T
