高度近视人群不同前房深度下水平睫状沟直径与角膜直径及前房角直径的相关性研究

摘　　要：目的探究高度近视眼在不同前房深度下水平睫状沟直径与角膜直径及前房角直径的相关性,从而分析能否通过外部测量方式来决定有晶状体眼人工晶状体的型号。方法收集2017年7~12月于安徽医科大学第二附属医院眼科行人工晶状体植入术前检查的高度近视患者,分别测量受试眼的角膜水平直径(WTW)、前房深度(ACD)、前房角水平直径(ATA)以及睫状沟水平直径(STS)。根据ACD将所有受检眼球分成浅前房(2.8 mm≤ACD<3.2 mm)、中前房(3.2 mm≤ACD≤3.4 mm)、深前房(ACD>3.4 mm)3个组进行分析,对3组内WTW、ATA、STS之间分别进行Pearson线性相关分析和线性回归方程分析。结果浅前房组,WTW与ATA及ATA与STS之间均存在显著性线性相关(P<0.01),WTW与STS之间具有的较弱相关性(P<0.05)。中前房组,WTW与ATA的相关性分析具有明显的统计学意义(P<0.01),WTW与STS及WTW与ATA之间的相关性分析无明显统计学意义。深前房组,WTW与ATA及ATA与STS之间均存在显著性线性相关(P<0.01),WTW与STS之间具有的较弱相关性(P<0.05)。结论用外部测量数据预测眼球内部解剖数据从而选择晶状体型号并不准确,将会增加因晶状体型号不适而带来的并发症发生率。Objective To investigate the correlation among the horizontal diameter of the ciliary sulcus,the horizontal diameter of the cornea and the horizontal diameter of the anterior chamber at different depths of the anterior chamber in high myopia patients,so as to investigate whether the model of the intraocular lens(IOL)can be determined by external measurement.Methods High myopia patients who underwent preoperative examination for IOL implantation in the Second Hospital of Anhui Medical University from July 2017 to December 2017 were enrolled.The corneal horizontal diameter of the test eyes(white-to-white,WTW),anterior chamber depth(ACD),angle-to-angle(ATA),and sulcus-to-sulcus(STS)of the eyes were measured.All eyes were divided into three groups based on the ACD:shallow anterior chamber group(2.8 mm≤ACD<3.2 mm),median anterior chamber group(3.2 mm≤ACD≤3.4 mm),and deep anterior chamber group(ACD>3.4 mm).Pearson linear correlation analysis and linear regression equation analysis were performed for WTW,ATA and STS in the three groups,and the WTW,ATA and STS among the three groups were compared by one-way ANOVA and SNK test.Results In the shallow anterior chamber group,there was a significant linear correlation between WTW and ATA,ATA and STS(P<0.01),and a weak correlation between WTW and STS(P<0.05).In the middle anterior chamber group,the correlation between WTW and ATA was statistically significant(P<0.01).However,there was no statistically significant association between WTW and STS or WTW and ATA.There was significant linear association between WTW and ATA,as well as between ATA and STS in the deep anterior chamber group(P<0.01),and a weak association between WTW and STS(P<0.05).Conclusions Using external measurement data to predict the internal anatomical data of the eye for IOL model selection is not accurate,which could increase the risk of surgical complications due to the unsuitable IOL.
