摘　　要：目的调查了解重症监护科室(ICU)护士工作压力源、睡眠状况与精神心理舒适度,并分析3者间的关系。方法以3家三级甲等医院重症监护科室中的86位女性护士为调查对象,采用基本资料调研问卷、护士工作压力源量表、躯体症状自评量表(symptom check list 90,SCL-90)、匹茨堡睡眠指数(Pittsburgh sleep index,PSQI)了解护士的工作压力、睡眠及精神心理状况,运用Pearson相关分析3者间的相关性,运用多元回归分析工作压力5个维度对护士精神心理状况和睡眠质量的影响。结果被调查护士工作压力源量表、SCL-90量表、PSQI量表总得分分别为(72.76±10.55)分、(139.32±15.37)分、(8.06±1.24)分。工作压力源总得分与PSQI(r=0.34)、SCL-90(r=0.49)呈正相关(P<0.05),SCL-90总得分与PSQI之间亦呈正相关(r=0.45,P<0.05)。护士的职业发展、工作环境与性质的回归系数满足统计学意义(P<0.05),共同解释睡眠指数的贡献率为12.7%。工作环境与性质、患者及家属态度、危重患者护理的回归系数满足统计学意义(P<0.05),共同解释躯体症状的贡献率为21.8%。结论护士的工作压力大,存在一定的精神心理问题,睡眠状况差,并且三者间互相影响。Objective To investigate the job stressors,sleep quality and mental state of intensive care unit(ICU)nurses,and analyze the relationships among them.Methods Eighty-six female nurses in the ICU of three Grade-A Tertiary hospitals were selected.Basic data questionnaire,nurse job stressors questionnaire(NJSQ),Pittsburgh sleep quality index(PSQI)and symptom checklist 90(SCL-90)were used to investigate the job stressors,sleep quality and mental state of ICU nurses.The relationships among them were analyzed by Pearson correlation analysis.Multiple regression analysis was used to analyze the influence of five dimensions of job stressors on mental state and sleep quality of nurses.Results The total scores of NJSQ,SCL-90 and PSQI were(72.76±10.55)points,(139.32±15.37)points and(8.06±1.24)points,respectively.The total score of NJSQ was positively correlated with PSQI(r=0.34),SCL-90(r=0.49)(P<0.05),and the total score of SCL-90 was also positively correlated with PSQI(r=0.45,P<0.05).The regression coefficients of career development,working environment and nature were statistically significant(P<0.05),and the contribution rate of PSQI was 12.7%.The regression coefficients of working environment and nature,attitude of patients and their families,and nursing of critical ill patients were statistically significant(P<0.05),and the contribution rate of SCL-90 was 21.8%.Conclusions ICU nurses have a lot of work pressure,some mental problems and poor sleep,and the three affect each other.
