摘　　要：目的比较接受隐形矫治器、钟摆矫治器矫治的安氏Ⅱ类牙性错[牙合]畸形患者牙列垂直向变化。方法选取安氏Ⅱ类牙性错[牙合]畸形患者40例,根据治疗时所用矫治器不同分为隐形组和钟摆组各20例,隐形组采用隐形矫治器矫治,钟摆组采用钟摆矫治器矫治,比较两组治疗前后上下颌第一磨牙(U6、L6)垂直向位移距离及上下颌骨骨性改变。结果磨牙远移后,隐形组U6 Y冠、Y根位移距离差值增加(P均<0.05)。与钟摆组比较,隐形组下颌第一磨牙近中颊尖的垂直向距离、前面下高度、前下面高与前面高比差值减小(P均<0.05)。结论相对于钟摆矫治器,隐形矫治器可以较好地控制安氏Ⅱ类牙性错[牙合]畸形患者磨牙远移后牙列的垂直向变化,推磨牙向远中时对上、下颌磨牙均有压低作用,磨牙远移时下颌角及下平面无明显增加,且面下1/3高度增加量相对较小。Objective To compare the vertical changes of dentition in patients with classⅡmalocclusion treated by clear aligner and pendulum appliance.Methods Forty patients with classⅡmalocclusion were enrolled.According to the different appliances used in the treatment,40 cases were divided into two groups:invisible group and pendulum group,with 20 in each.The vertical displacement distance of the first molars(U6,L6)in maxilla and mandible and the skeletal changes of the mandible were measured by Dolphin Imaging software before and after treatment.Results The difference of displacement distance between maxillary first molar crown and root increased significantly in the invisible group(P<0.05).Compared with the pendulum group,the differences of vertical distance of mandible first molars(LDPH),lower facial height(LFH),and the rate of lower facial height and anterior facial height(ANS-Me:N-Me)decreased in the invisible group(all P<0.05).Conclusions Compared with pendulum appliance,clear aligner can better control the vertical change of the dentition of patients with classⅡmalocclusion.When we put the molars toward the middle distance,it can depress the upper and lower molars.There is no significant increase in the mandibular angle and the lower plane,and the 1/3 height increase is relatively small.
