岩黄连注射液联合TACE对中晚期肝癌患者炎性反应、氧化应激的影响

摘　　要：目的观察岩黄连注射液联合TACE对中晚期肝癌患者炎性反应、氧化应激和相关因子的影响。方法选取2016年1月至2019年1月收治的中晚期肝癌患者110例,随机分为对照组和治疗组,每组55例。2组均给予TACE治疗,治疗组在TACE基础上加用岩黄连注射液治疗。观察2组外周血单核细胞(PBMCs)Toll样受体4(TLR4)表达情况及血清炎性因子、氧化应激指标、肿瘤活性相关指标、肿瘤复发相关指标变化。结果2组治疗前PBMCs TLR4阳性细胞表达百分率及血清CRP、TNF-α、PCT、SOD、MDA、VEGF、MMP-9、GP-73、GPC-3、AFP-L3、GGT比较差异无统计学意义(P>0.05)。2组治疗后PBMCs TLR4阳性细胞表达百分率及血清CRP、TNF-α、PCT、MDA、VEGF、MMP-9、GP-73、GPC-3、AFP-L3、GGT均明显低于治疗前,SOD均明显高于治疗前(P<0.05),且治疗组TLR4阳性细胞表达百分率、CRP、TNF-α、PCT、MDA、VEGF、MMP-9、GP-73、GPC-3、AFP-L3、GGT低于对照组,SOD高于对照组(P<0.05)。结论岩黄连注射液联合TACE治疗中晚期肝癌疗效确切,能够显著下调PBMCs TLR4的表达,抑制炎性应激反应,并降低血清相关因子水平。Objective To investigate the effects of Yanhuanglian injection combined with transcatheter arterial chemoembolization(TACE)on the inflammatory reaction,oxidative stress and related factors of patients with advanced liver cancer.Methods A total of 110 patients with middle and advanced liver cancer were enrolled in the study,who were divided into observation group and control group,with 55 cases in each group.The patients in both groups were treated by TACE,on that basis,the patients in observation group were treated by Yanhuanglian injection.The expression levels of Toll-like receptor 4(TLR4)in peripheral blood monocytes(PBMCs)and the serum levels of inflammatory cytokines,oxidative stress indexes,tumor activity related indicators and tumor recurrence related indicators were observed and compared between the two groups.Results Before treatment,there were no significant differences in the expression levels of TLR4 in PBMCs and the serum levels of CRP,TNF-α,PCT,SOD,MDA,VEGF,MMP-9,GP-73,GPC-3,AFP-L3 and GGT between the two groups(P>0.05).After treatment,the expression levels of TLR4 in PBMCs and the serum levels of CRP,TNF-α,PCT,MDA,VEGF,MMP-9,GP-73,GPC-3,AFP-L3 and GGT were significantly decreased in both groups,however,the SOD levels were significantly increased in both groups(P<0.05).After treatment,the expression levels of TLR4 in PBMCs and the serum levels of CRP,TNF-α,PCT,MDA,VEGF,MMP-9,GP-73,GPC-3,AFP-L3 and GGT in observation group were significantly lower than those in control group,but,the SOD levels in observation group were significantly higher than those in control group(P<0.05).Conclusion Yanhuanglian injection combined with TACE in treatment of middle and advanced liver cancer can effectively down-regulate the expressions of TLR4 in PBMCs,inhibit inflammatory stress and reduce the serum levels of cytokines.
