甲磺酸阿帕替尼联合放疗治疗局部晚期胰腺癌的临床观察

摘　　要：目的研究胰腺癌患者采用甲磺酸阿帕替尼联合放射治疗的临床疗效与安全性。方法选取2017年6月至2019年6月收治的60例初治的局部晚期胰腺癌患者,随机分为联合组和单放组,每组30例。联合组采用甲磺酸阿帕替尼联合三维适形放疗,单放组单纯采用三维适形放疗,对比分析2组的治疗效果及不良反应发生情况。结果联合组患者的肿瘤局部控制率(RR)显著高于单放组,差异有统计学意义(P<0.05)。2组患者不良反应发生率比较,差异无统计学意义(P>0.05)。联合组与单放组1、2年生存率分别为66%、29%和38%、15%,中位生存期分别为16个月和11个月(χ^2=4.21,P<0.05),有效率分别为64%和32%(χ^2=5.22,P<0.05)。结论甲磺酸阿帕替尼同步联合三维适形放疗治疗胰腺癌,有助于提高胰腺癌患者的局部控制率,且治疗相关不良反应轻,是治疗局部晚期胰腺癌有价值的治疗方案。Objective To investigate the therapeutic effects and safety of apatinib combined with radiotherapy in treatment of local advanced pancreatic cancer.Methods A total of 60 patients with local advanced pancreatic cancer who were treated in our hospital from June 2017 to June 2019 were randomly divided into combination treatment group and the single radiotherapy group,with 30 cases in each group.The patients in combination treatment group were treated by apatinib combined with three-dimensional conformal radiotherapy,however,the patients in single radiotherapy group were treated by three-dimensional conformal radiotherapy only.The therapeutic effects and adverse reactions were observed and compared between the two groups.Results The local tumor control rate(RR)in combination treatment group was significantly higher than that in single radiotherapy group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two group(P>0.05).The 1-year and 2-year survival rates were 66%,29%in combination treatment group and 38%,15%in single radiotherapy group,respectively.The median survival time was 16 months and 11 months in combination treatment group and single radiotherapy group,respectively(χ^2=4.21,P<0.05),and the effective rates were 64%and 32%in combination treatment group and single radiotherapy group,respectively(χ^2=5.22,P<0.05).Conclusion The apatinib combined with three-dimensional conformal radiotherapy in treatment of pancreatic cancer is helpful to improve the local control rate of patients in patients with pancreatic cancer,and the treatment-related adverse reactions are mild,which is a valuable treatment scheme for local advanced pancreatic cancer.
