不同乳突切除术联合鼓室成形术治疗慢性化脓性中耳炎的效果及对听力的影响

摘　　要：目的分析慢性化脓性中耳炎患者采用不同乳突切除术联合鼓室成形术对听力的影响。方法选取2017年5月至2018年5月该院耳鼻喉科接诊的85例慢性化脓性中耳炎患者作为研究对象,掷币法分组,对照组42例接受开放式乳突切除术联合鼓室成形术,研究组43例接受完璧式乳突切除术联合鼓室成形术,比较两组患者治疗效果及听力水平。结果研究组干耳所需时间短于对照组,差异有统计学意义(P<0.05)。研究组总有效率高于对照组,纯音听力、气导听阈及骨导听阈均低于对照组,差异有统计学意义(P<0.05)。结论完璧式乳突切除术联合鼓室形成术治疗慢性化脓性中耳炎效果显著,能显著改善患者听力,缩短干耳时间,最大限度保留外耳道结构,减少术后并发症发生,值得临床推荐。【Objective】To analyze the effect of different mastoidectomy combined with tympanoplasty on hearing in patients with chronic suppurative otitis media.【Methods】A total of 85 patients with chronic suppurative otitis media who were admitted to the department of otorhinolaryngology of our hospital from May 2017 to May 2018 were enrolled in the study,and they were divided into two groups by coin toss method.Forty-two patients in the control group underwent open mastoidectomy combined with tympanoplasty.Forty-three patients in the study group underwent intact canal wall mastoidectomy combined with tympanoplasty,and the therapeutic effects and hearing levels of the two groups were compared.【Results】The time required for dry ear in the study group was shorter than that in the control group,and the difference was significant(P<0.05).The total effective rate of the study group was higher than that of the control group;the pure tone hearing,air conduction hearing threshold and bone conduction hearing threshold were lower than the control group,and the difference was significant(P<0.05).【Conclusion】Intact canal wall mastoidectomy combined with tympanoplasty is effective in the treatment of chronic suppurative otitis media.It can significantly improve the hearing of patients,shorten the time of dry ear,maximize the structure of external auditory canal,and reduce postoperative complications.It is worthy of clinical recommendation.
