摘 要:目的系统评价去除菌斑生物膜后局部使用盐酸米诺环素对种植体周围炎的疗效,为盐酸米诺环素在种植体周围炎治疗中的作用提供循证医学证据。方法检索数据库:中国知网、万方、维普、中国生物医学文献数据库、PubMed和Cochrane Library等;检索时限:建库至2019年3月。由2位研究者独立地鉴别、分析、评估相关文献后使用Rev Man 5.3以及Stata 12.0进行数据分析。结果包含637个种植体的7个随机对照试验被最终纳入。Meta分析数据表明,盐酸米诺环素组与对照组探诊深度(PD)、出血指数(SBI)、菌斑指数(PI)的变化有差异[MD=-0.64,95%CI(-1.14,-0.14),P=0.01]、[MD=-2.12,95%CI(-3.09,-1.15),P<0.01]、[MD=-0.73,95%CI(-1.19,-0.28),P=0.002],且差异有统计学意义。结论盐酸米诺环素对减少种植体周围PD、SBI、PI及感染控制方面有明显的效果。Objective To evaluate the efficacy of minocycline hydrochloride in the treatment of peri-implantitis,and to provide evidence-based medicine for the treatment of peri-implantitis with minocycline hydrochloride.Methods The database we used concluded CNKI,VIP,Chinese Biomedical Literature,PubMed,Cochrane Library from inception to March 2019.The literatures were screened by two reviewers independently.Then,meta-analysis was performed using Rev Man 5.3 and Stata12.0.Results Seven randomized controlled trials were selected which involved 637 dental implants.Meta-analysis showed that the changes in Probing depth(PD)[MD=-0.64,95%CI(-1.14,-0.14),P=0.01],Sulcus bleeding index(SBI)[MD=-2.12,95%CI(-3.09,-1.15),P<0.01]and plaque index(PI)[MD=-0.73,95%CI(-1.19,-0.28),P=0.002]had significant statistical difference between minocycline hydrochloride group and control group.Conclusion In the short-term observation,the local application of minocycline hydrochloride plays an effective role in the therapy of peri-implantitis.
