摘　　要：目的探讨白细胞介素-26(IL-26)在结直肠癌(CRC)患者外周血中的表达及其在鉴别诊断CRC和直肠息肉(CRP)中的价值。方法选取2017年8月至2019年12月在广东省第二人民医院住院治疗的80例CRC患者作为研究对象,依据第8版美国肿瘤联合会CRC TNM分期系统将研究对象分为临床Ⅰ期(n=15)、Ⅱ期(n=25)、Ⅲ期(n=25)和Ⅳ期(n=15)等4组。同时入选CRP患者作为对照组(n=30)。收集研究对象临床资料,酶联免疫吸附法检测血清IL-26表达。ROC曲线分析血清IL-26水平对CRC的诊断价值。结果与对照组相比,血清IL-26水平在临床Ⅰ期、Ⅱ期、Ⅲ期和Ⅳ期等各组中均表达升高,并呈依次增高趋势(P<0.01)。Spearman相关分析显示,血清IL-26、CEA、CA199等水平分别与CRC临床分期呈正相关(P<0.01)。血清IL-26预测CRC转移的ROC曲线下面积最大,为0.940(95%CI:0.890-0.990),敏感性为0.975,特异性为0.8。结论IL-26在CRC患者外周血表达升高,与CRC的临床分期密切相关,可作为CRC与CRP鉴别诊断的标记分子。Objective To investigate the expression of interleukin-26( IL-26) in peripheral blood of patients with colorectal cancer( CRC) and its value in differential diagnosis of CRC and colorectal polyps( CRP). Methods 80 patients with CRC who were hospitalized in the Guangdong second provincial general hospital from August 2017 to December 2019 were enrolled in the research.According to the TNM staging system of the 8 th edition of the American Cancer Association,the research objects were divided into four groups: clinical stage Ⅰ( n = 15),stage Ⅱ( n = 25),stage Ⅲ( n = 25) and stage Ⅳ( n = 15). Patients with CRP were enrolled into the control group( n = 30). The clinical data of the subjects were collected and the expression of IL-26 in serum was detected by enzyme-linked immunosorbent assay. ROC curve analysis of serum IL-26 levels was used in the differential diagnosis of CRC and CRP.Results Compared with the control group,the level of serum IL-26 increased in clinical stages Ⅰ,Ⅱ,Ⅲ and Ⅳ,and showed a trend of increasing in turn( P < 0. 01). Spearman correlation analysis showed that serum levels of IL-26,CEA and CA199 were positively correlated with clinical stages of colorectal cancer( P < 0. 01). The area under ROC curve of serum IL-26 in predicting colorectal cancer metastasis was the largest,which was 0. 940( 95% CI: 0. 890-0. 990),with a sensitivity of 0. 975 and a specificity of 0. 8.Conclusion The expression of IL-26 in the peripheral blood of patients with CRC is increased,which is closely related to the clinical stage of CRC,can be used as a marker for differential diagnosis of CRC and CRP.
