摘  要:目的:探讨实时剪切波弹性成像技术(SWE)对甲状腺恶性肿瘤的应用诊断价值。方法:选择2017年1月-2019年6月常规超声检查诊断单侧甲状腺恶性肿瘤患者108人,采用SWE技术对其单侧甲状腺结节及对侧正常甲状腺组织进行检查,均测量剪切波速度(SWV)及杨氏模量值(EI)。追踪患者病理手术结果,终31例患者(40例恶性肿瘤)资料完整。以患者术后病理结果作为最终判定标准,统计分析不同病变及正常组织间SWV及EI的差异;比较常规超声检查与SWE技术的准确性、灵敏性及特异性。结果:常规超声与SWE技术诊断准确性比较无统计学意义,二者联合检查与常规超声检查相比准确性高,差异有统计学意义(P<0.05)。恶性结节SWV及EI高于良性结节(P<0.05)及正常腺体明显(P<0.05)。结论:甲状腺肿瘤恶性肿瘤较良性肿瘤、正常甲状腺组织的剪切波速度及杨氏模量增大;常规超声和SWE技术联合运用,能提高甲状腺恶性肿瘤的诊断准确性。Objective To explore the value of real-time shear wave elastography (SWE) in diagnosing thyroid malignancy.Methods We inspected 108 Patients with unilateral thyroid nodules by the conventional ultrasound technology during 2017.1~2019.6,and recorded the shear wave velocity (SWV) and the young’s modulus value (EI).We also inspected the other side of thyroid tissue which is the normal thyroid tissue,measuring SWV and EI,took the patients’ postoperative pathologic results as the final judgement standard,analyzed the difference of elastic parameters between pathological and normal tissues,It also compares the sensitivity,specificity and veracity of routine ultrasonic and real-time SWE technology.Results Diagnosis of thyroid malignancy by routine ultrasound:sensitivity 63.64%,specificity 61.11%,veracity 62.5%;The sensitivity of SWE technology was 78.57%,specificity 62.50% and veracity 80.0%.The sensitivity of the two inspection(joint detection)was 82.14%,specificity 66.67% and veracity 82.5%.There was no statistical significance between routine ultrasound and SWE technology diagnosis.The veracity of joint inspection is higher than the routine ultrasound inspection,the difference was statistically significant(P<0.05).2.The record of SWV and EI in malignant nodules was obviously higher than normal thyroid tissue,(P<0.05),The record of SWV and EI in malignant nodules was higher than benign nodules too.Conclusion The SWV and Young’s modulus in malignant is increased than benign nodular and normal thyroid tissue;The routine ultrasound and SWE combination technology(joint detection) could improve the diagnostic veracity of thyroid malignant tumor.
