摘　　要：目的:评估临床药师会诊效果,为临床药学工作持续改进提供参考。方法:对2016–2018年临床药师参与3847例会诊进行回顾性分析,统计申请会诊科室、会诊类型、建议采纳等情况,对会诊典型案例进行解析。结果:会诊数量及会诊意见采纳率逐年增加。会诊目的以抗感染治疗用药为主,会诊意见完全采纳率85.4%,采纳建议后临床治疗有效率为89.1%。会诊过程中,一例罕见嗜沫凝聚杆菌感染性心内膜炎患者参照药敏治疗效果欠佳,经临床药师会诊后患者病情好转,对该病例进行深入解析。结论:临床药师会诊工作正逐步得到临床认可,需进一步在会诊工作制度化以及标准化方面做出努力。Objective: To evaluate the consultation effect of clinical pharmacists, and provide references for the continuous improvement of clinical pharmacy. Methods: 3847 cases of consultation of clinical pharmacists from 2016 to 2018 were systematically reviewed and analyzed in terms of application division, consultation type, and adoption ratio of advice and so on. And a typical case was simultaneously discussed. Results: The number of consultations and the adoption rate increased year by year. Rational use of anti-infective drugs was the main purpose of consultation. The adoption rate of clinical pharmacists’ advice was 85.4%. And the clinical effective rate was 89.1% after taking clinical pharmacists’ advice. A rare case of infective endocarditis caused by Aggregatibacter aphrophilus was discussed. The curative effect was poor with drugs selected according to the drug sensitivity test. And the patient was improved after consultation of clinical pharmacist. Conclusion: The consultation work of clinical pharmacists is gradually getting clinical recognition, and further efforts should be made in the institutionalization and standardization of consultation work.
