注射用脂溶性维生素(Ⅱ)/注射用水溶性维生素组合包装致过敏性休克1例

摘　　要：1例37岁足月孕妇,否认药物和食物过敏史,无心脏病史,家族史和个人史无特殊。因高龄初产,行子宫下段剖宫产术,手术顺利。术后为补充营养及能量,遵医嘱予5%葡萄糖注射液500 mL+注射用脂溶性维生素(Ⅱ)/注射用水溶性维生素组合包装1盒静脉滴注。输入约5 min后患者出现胸闷、呼吸困难,P 105次·min^-1,R 35次·min^-1,BP 72/37 mm Hg,血氧饱和度88%,考虑为过敏性休克。立即停止输入该药,给予吸氧、0.9%氯化钠注射液双通道扩容、盐酸肾上腺素注射液1 mg、地塞米松磷酸钠注射液20 mg抢救治疗。维持循环稳定,动态监测心电图,同时转入重症医学科治疗,1 d后患者血压、脉搏、血氧饱和度恢复正常,神志清醒,病情稳定。One 37-year-old full-term pregnant woman was admitted for lower segment cesarean section because she was an elderly primipara.The patient had no allergic history of drug and food,no history of heart disease,and no special family and personal history.The surgery was successful.And fat-soluble vitamin(Ⅱ)for injection/water-soluble vitamin for injection collective packing solved in 500 mL 5%glucose injection was intravenously given to her for nutrition and energy supplement.5 minutes later,the patient developed chest tightness and dyspnea.The pulse was 105 times·min^-1.The breath rate was 35 times·min^-1.The blood pressure was 72/37 mm Hg.And the blood oxygen saturation was 88%.Anaphylactic shock was diagnosed,and fat-soluble vitamin(Ⅱ)for injection/water-soluble vitamin for injection collective packing was stopped at once.Rescue treatments including oxygen inhalation,double channel expansion of 0.9%sodium chloride injection,adrenaline hydrochloride injection 1 mg and dexamethasone sodium phosphate injection 20 mg were given to her at the same time.And the patient was transferred to intensive care unit(ICU)with the stable circulation and dynamic monitoring of electrocardiogram(ECG).One day later,her blood pressure,pulse and blood oxygen saturation returned to normal.The patient was conscious and her condition was stable.
