海南省综合医院精神科联络会诊临床分析

摘　　要：目的探讨海南省综合医院精神科联络-会诊精神医学服务现状及存在问题.方法对海南医学院第二附属医院精神科开展联络会诊服务的146例患者的临床资料进行统计分析,内容包括人口学资料、申请会诊科室、精神科会诊后再诊断情况、因躯体疾病伴发/单纯精神疾病申请会诊等.结果本组患者年申请会诊率为0.03%;男、女比例相同,年龄分别为(55.23±15.79)岁、(57.56±16.66)岁;申请会诊率内科(58.9%)高于外科(34.2%),内科以老年科(15.8%)、心内科(9.6%)、内分泌科(8.9%)为多,外科以妇科(8.9%)、心血管外科(7.5%)、肿瘤外科(7.5%)、骨科(6.2%)为多;精神科会诊后再诊断以睡眠障碍(以失眠为主)(28.1%)、焦虑障碍/焦虑状态(25.3%)、器质性精神障碍(15.8%)、抑郁障碍/抑郁状态(13.0%)、精神分裂症(8.2%)为多;因躯体疾病伴发/单纯精神疾病申请会诊以睡眠障碍(以失眠为主)(13.7%)、精神病性障碍(11.0%)、焦虑障碍(10.3%)、癌性病变伴发精神异常(9.6%)、脑梗死/脑出血后伴发精神异常(8.9%)为多.结论综合性医院普通临床科室有较大精神科会诊需求,非精神科医师对抑郁症状的识别力有待提高.Objective To discuss the present situation and existing problems of liaison consultation psychiatric service in the new psychiatric department of a general hospital in Hainan province.Methods The clinical data of 146 patients with liaison consultation service in the psychiatry department of the Second Affiliated Hospital of Hainan Medical College was analyzed.The content includes demographic information,consultation department application,re diagnosis after psychiatric consultation,and application for consultation due to physical illness accompanying/simple mental illness.Results The annual consultation rate for this group of patients was 0.03%;the proportion of males and females was the same,and the ages were(55.23±15.79)years and(57.56±16.66)years.The application rate for consultation was higher in internal medicine(58.9%)than in surgery(34.2%).The majority of internal medicine was geriatrics(15.8%),cardiology(9.6%),and endocrinology(8.9%).The majority ofsurgery was gynecology(8.9%),cardiovascular surgery(7.5%),oncology surgery(7.5%),orthopedics(6.2%).Re diagnosis after consultation in the psychiatric department,mainly were sleep disorders(mainly insomnia)(28.1%),anxiety disorder/anxiety state(25.3%),organic mental disorder(15.8%),depressive disorder/depressive state(13.0%)and schizophrenia(8.2%).Application for consultation due to physical ilness accompanied by simple mental illness,mainly were sleep disorders(mainly insomnia)(13.7%),psychotic disorders(11.0%),anxiety disorders(10.3%),cancerous disorders accompanied by mental disorders(9.6%),psychiatric disorders after cerebral infarction/intracerebral hemorrhage(8.9%).Conclusions General clinical departments in general hospitals have a large demand for psychiatric consultation,and the recognition of depression symptoms by non-psychiatrists needs to be improved.
