胸部增强CT联合双时相18F-FDG PET/CT显像对肺炎型肺癌的诊断价值

摘　　要：目的 观察胸部增强CT联合双时相18F-FDG PET/CT显像对肺炎型肺癌的诊断价值。方法 采用前瞻性随机双盲对照试验法,选择2016年1月至2019年12月阜阳市第二人民医院收治的67例肺炎型肺癌患者,以患者的病理学结果为金标准,对比胸部增强CT、单时相PET/CT、胸部增强CT联合单时相PET/CT、双时相18F-FDG PET/CT、胸部增强CT联合双时相18F-FDG PET/CT诊断肺炎型肺癌的正确率。结果 胸部增强CT、单时相PET/CT、胸部增强CT联合单时相PET/CT、双时相18F-FDG PET/CT、胸部增强CT联合双时相18F-FDG PET/CT诊断肺炎型肺癌的正确率分别为26. 87%(18/67)、62. 69%(42/67)、77. 61%(52/67)、71. 64%(48/67)、91. 04%(61/67);经χ2检验,差异具有统计学意义(P <0. 05);经Fisher确切概率法检验,胸部增强CT、胸部增强CT联合双时相18F-FDG PET/CT诊断肺炎型肺癌的正确率比较,差异具有统计学意义(P <0. 05),其余组间比较差异无统计学意义(P> 0. 05)。结论 胸部增强CT联合双时相18F-FDG PET/CT显像对肺炎型肺癌的诊断正确率高于胸部增强CT、单时相PET/CT、胸部增强CT联合单时相PET/CT、双时相18F-FDG PET/CT,表现出较高的临床应用价值。Objective To observe the diagnostic value of contrast enhanced chest CT combined with dual-phase 18 F-FDG PET/CT imaging in the patients with pneumonia type pulmonary carcinoma.Methods With prospective,randomized,double-blind controlled experiment method,67 cases of patients with pneumonia type pulmonary carcinoma from January 2016 to December 2019 in No.2 People's Hospital of Fuyang city were selected in this experiment.With pathology result as the gold standard,the diagnostic accuracy of contrast enhanced chest CT,single phase PET/CT,contrast enhanced chest CT with single phase PET/CT,dual-phase PET/CT,contrast enhanced chest CT with dual-phase PET/CT was compared.Results The accuracy rates of the examination methods referred above in diagnosing pneumonitis lung cancer were 26.87%(18/67),62.69%(42/67),77.61%(52/67),71.64%(48/67)and 91.04%(61/67),respectively.Chi-square test showed a statistically significant difference(P<0.05),Fisher's exact probability test showed a statistically significant difference in the accuracy of contrast enhanced chest CT and contrast enhanced chest CT combined with dual-phase 18 F-FDG PET/CT in the diagnosis of pneumonia type pulmonary carcinoma(P<0.05),but no statistically significant difference was found among the other groups(P>0.05).Conclusion Chest-enhanced CT combined with dual-phase 18 F-FDG PET/CT imaging is more accurate in the diagnosis of pneumonia-type lung cancer than chest-enhanced CT,single-phase PET/CT,chest-enhanced CT combined with single-phase PET/CT,and dual-phase 18 F-FDG PET/CT shows high clinical application value.
