摘  要:目的观察非小细胞肺癌(NSCLC)脑转移患者的CEA、CYFRA21-1、SCC-Ag水平,并分析其临床意义。方法选取2017年3月~2018年8月在首都医科大学附属北京潞河医院接受治疗的NSCLC患者120例,并根据其是否发生脑转移分为脑转移组(45例)和未发生脑转移组(75例)。观察两组患者CEA、CYFRA21-1、SCC-Ag水平的差异,比较不同临床病理特征NSCLC脑转移患者的CEA、CYFRA21-1、SCC-Ag水平,并分析影响NSCLC脑转移患者的CEA、CYFRA21-1、SCC-Ag水平的因素。结果NSCLC脑转移患者的CEA、CYFRA21-1、SCC-Ag水平分别为(3.46±1.01)ng/ml、(9.98±1.55)ng/ml、(18.02±3.06)ng/ml,高于未发生脑转移的单纯NSCLC组(2.03±0.69)ng/ml、(2.15±0.82)ng/ml、(6.85±1.14)ng/ml,差异有统计学意义(t=-8.389、-31.324、-23.495,P<0.001)。将单因素分析有意义的因素作为因变量,将NSCLC脑转移患者的CEA、CYFRA21-1、SCCAg水平作为自变量进行回归分析,结果显示分化程度是影响NSCLC脑转移患者CEA、CYFRA21-1、SCC-Ag水平的因素。结论NSCLC患者发生脑转移患者的血清CEA、CYFRA21-1、SCC-Ag水平较高,且与患者的分化程度密切相关。Objective To observe the levels of serum CEA,CYFRA21-1 and SCC-Ag in patients with NSCLC brain metastases,and analyze their clinical significance.Methods The patients with NSCLC who were treated in our hospital from March2017 to August 2018 were selected as the research subjects,and they were divided into brain metastasis group and non-brain metastasis group according to whether brain metastasis occurred.Observe the differences in serum CEA,CYFRA21-1,and SCC-Ag levels in the two groups of patients,compare the CEA,CYFRA21-1,and SCC-Ag levels in patients with NSCLC brain metastases with different clinicopathological characteristics,and analyze the CEA and CYFRA21-1.Factors of SCC-Ag level.Results The levels of serum CEA,CYFRA21-1,and SCC-Ag in patients with NSCLC brain metastases were(3.46±1.01)ng/ml,(9.98±1.55)ng/ml,and(18.02±3.06)ng/ml,which were higher than those in the NSCLC group alone(t=-8.389,-31.324,-23.495,P<0.001).the factors that were significant in univariate analysis were used as dependent variables,and the CEA,CYFRA21-1,and SCC-Ag levels in patients with NSCLC brain metastases were used as the dependent variables.Regression analysis of independent variables showed that the degree of differentiation was a factor that affected CEA,CYFRA21-1,and SCC-Ag levels in patients with NSCLC brain metastases.Conclusion Serum CEA,CYFRA21-1,and SCC-Ag levels are higher in patients with brain metastases in NSCLC patients,and are closely related to the degree of differentiation of patients.
