微创血肿清除术治疗HICH患者的效果观察

摘　　要：探讨采用微创血肿清除术治疗高血压脑出血(HICH)患者的临床效果。选取2016年2月至2017年12月本院收治的110例HICH患者,其中55例患者采用微创血肿抽吸术治疗(微创组)、另外55例采用传统开颅血肿清除术治疗(对照组),比较两组患者的手术时间、血肿清除率、住院费用、住院时间,术前、术后7天、术后14天的血清S100B蛋白、神经元特异性烯醇化酶(NSE)、白细胞介素-6(IL-6)水平,术前、术后7天、术后14天神经缺损功能评分(NIHSS)及手术并发症。结果显示微创组手术时间低于对照组(P<0.05),但微创组术后3天的血肿清除率、住院费用、住院时间均低于对照组(P<0.05);术后7天、术后14天,微创组血清S100B蛋白、神经元特异性烯醇化酶(NSE)、白细胞介素-6(IL-6)水平、NIHSS评分均低于对照组(P<0.05);且手术并发症发生率(10.91%)低于对照组(25.45%)(P<0.05)。采用微创血肿清除术治疗HICH患者具有手术时间短、创伤小,术后患者神经功能康复效果较好的特点,同时手术并发症发生率低。To investigate the clinical effect and neurological function of HICH patients treated with minimally invasive hematoma removal.110 patients with HICH were selected,55 of them were treated with minimally invasive hematoma aspiration(minimally invasive group),and another 55 were treated with traditional craniotomy hematoma clearance(control group).The operation time of the minimally invasive group was higher than that of the control group(P<0.05),and the hematoma clearance rate,hospitalization cost and hospitalization time of the minimally invasive group were lower than those of the control group(P<0.05).The levels of serum S100B protein,neuron specific enolase(NSE)and interleukin-6(IL-6)in the minimally invasive group were lower than those in the control group 7 days and 14 days after operation(P<0.05).The NIHSS score of the minimally invasive group was lower than that of the control group(P<0.05).The operative complication rate of the minimally invasive group was 10.91%lower than that of the control group(25.45%(P<0.05)).The results showed that minimally invasive hematoma removal for HICH patients had the characteristics of short operation time,less trauma,better neurological rehabilitation effect and low complication rate.
