摘 要:目的探讨乳腺病变中异型大汗腺增生的形态学变化及其意义。方法复习本院收治的1例伴单纯性大汗腺分化的浸润性乳腺癌的组织形态,并进行相关免疫组化染色观察。结果镜下可见大汗腺化生呈囊肿、乳头状增生、不典型增生、低级别导管原位癌和浸润性大汗腺癌的形态改变,肿瘤细胞弥漫表达AR和GCDFP-15。结论大汗腺化生广泛存在于乳腺的良性病变中,恶性病变中相对罕见,恒定表达AR和GCDFP-15可以证实大汗腺化生;因其细胞形态和组织结构的特殊性,应提高对异型大汗腺增生的组织形态认识,避免漏诊。Objective To observe the morphologic changes and significance of apocrine metaplasia with atypia in breast lesions.Methods The morphological characteristics were reviewed in a case of invasive breast carcinoma with pure apocrine differentiation under microscope in our hospital;immunohistochemical stains were done and analyzed.Results Apocrine metaplasia exhibited cystic,papillary,atypical type,low grade apocrine ductal carcinoma in situ and invasive carcinoma with apocrine differentiation under microscope.Immunohistochemical stains revealed that AR and GCDFP-15 were positive in apocrine metaplasia lesions.Conclusion Apocrine metaplasia extensively exists in the benign lesions of the breast,but rare appears in malignant lesions,and can be identified by expression of AR and GCDFP-15.Because of the particularity of cell morphology and tissue structure,we should improve the understanding of the histomorphology of apocrine metaplasia with atypia and avoid missed diagnosis.
