摘 要:目的探讨18F-FDG PET/CT显像代谢参数在鼻咽癌诊断分期及预测其转移中的应用价值。方法回顾性分析50例经病理证实的鼻咽癌患者的PET/CT资料,其中转移组35例,无转移组15例;早期鼻咽癌(Ⅰ-Ⅱ期)8例,进展期(Ⅲ-Ⅳ期)42例。分别测量其原发病灶的PET/CT代谢参数:最大标准摄取值(SUVmax)、平均标准摄取值(SUVmean)、峰值标准摄取值(SUVpeak)、病灶糖酵解总量(TLG)和肿瘤代谢体积(MTV)。比较转移组与无转移组,早期与进展期患者间一般资料、代谢参数的差异,并对有统计学意义的参数绘制ROC曲线,计算曲线下面积(AUC),并对结果进行分析。结果转移组与无转移组原发灶TLG、MTV差异有统计学意义(P均<0.05),性别、年龄、SUVmax、SUVpeak、SUVmean差异无统计学意义(P均>0.05)。ROC曲线分析中,TLG(AUC为0.817)的预测转移效能优于MTV(AUC为0.783)。早期鼻咽癌(Ⅰ-Ⅱ期)与进展期(Ⅲ-Ⅳ期)原发灶MTV差异有统计学意义(P<0.05),在性别、年龄、SUVmax、SUVpeak、SUVmean、TLG上差异无统计学意义(P均>0.05)。ROC曲线分析中,MTV(AUC为0.771)预测临床分期有一定准确性。结论 PET/CT代谢参数TLG、MTV对预测鼻咽癌转移具有一定价值,MTV与鼻咽癌临床分期有一定关系,利用MTV,18F-FDG PET/CT可以预测鼻咽癌的临床分期。Objective To investigate the application value of 18F-FDG PET/CT imaging parameters in the diagnosis and prediction of nasopharyngeal carcinoma.Methods The PET/CT data of 50 patients with pathologically confirmed nasopharyngeal carcinoma were retrospectively analyzed,including 35 patients in the metastasis group and 15 patients in the non-metastasis group.Eight patients with early nasopharyngeal carcinoma (stage Ⅰ-Ⅱ),advanced stage (Ⅲ-Stage Ⅳ) 42 cases.The PET/CT metabolic parameters of the primary lesion were measured:maximum standard uptake value (SUV max),mean standard uptake value (SUVmean),peak standard uptake value (SUVpeak),total glycolytic level (TLG),and tumor metabolic volume.(MTV).The differences between general data and metabolic parameters between the early and the advanced patients were compared between the metastatic group and the non-metastasis group,and the ROC curves were plotted for the statistically significant parameters.The area under the curve (AUC) was calculated and the results were analyzed.Results There were significant differences in TLV and MTV between the metastatic group and the non-metastasis group (P<0.05).There was no significant difference in gender,age,SUVmax,SUVpeak and SUVmean (P>0.05).In the ROC curve analysis,the predicted transfer efficiency of TLG (AUC 0.817) was better than MTV (AUC was 0.783).There were significant differences in MTV between early nasopharyngeal carcinoma (stage Ⅰ-Ⅱ) and advanced stage (stage Ⅲ-Ⅳ)(P<0.05).There were no differences in gender,age,SUVmax,SUVpeak,SUVmean and TLG.Academic significance (P>0.05).In ROC curve analysis,MTV (AUC 0.771) predicts clinical stage with certain accuracy.Conclusion PET/CT metabolic parameters TLG and MTV have certain value in predicting metastasis of nasopharyngeal carcinoma.MTV has a certain relationship with clinical stage of nasopharyngeal carcinoma.MTV and 18F-FDG PET/CT can predict the clinical stage of nasopharyngeal carcinoma.
