DOG1抗体新克隆MX067在胃肠间质瘤病理诊断应用中的初步评价

摘　　要：目的对DOG1抗体新克隆MX067在胃肠间质瘤(GIST)病理诊断应用进行初步评价。方法收集71例GIST组织和79例非GIST肿瘤组织(乳腺癌、胃癌、神经鞘瘤和结肠癌等),制成组织芯片分别进行DOG1鼠单抗MX067、K9及兔单抗SP31免疫组化染色及分析对比。结果 DOG1 MX067、K9和SP31抗体在GIST中的阳性检出率分别为90. 14%(64/71)、88. 73%(63/71)及88. 73%(63/71),DOG1 MX067抗体在GIST中的阳性检出率与K9、SP31抗体无显著性差异(P>0. 05)。DOG1 SP31在神经鞘瘤组织中出现8. 82%(3/34)的阳性反应,而DOG1MX067和DOG1 K9为阴性。DOG1 MX067抗体检测诊断GIST的敏感性高于DOG1 K9、SP31,DOG1 MX067特异性等于K9,高于DOG1 SP31,DOG1 MX067 ROC曲线下面积0. 926大于DOG1K9、SP31。结论克隆号为MX067的DOG1抗体特异性强,敏感性高,对于GIST的诊断有较高的临床应用价值,建议推广和应用于GIST的病理诊断。Objective To evaluate new monoclonal anti DOG1( discovered on GIST-1) MX067 for the diagnosis of gastrointestinal stromal tumors( GIST). Methods Tissue microarrays were made with 71 cases of GIST and 79 cases of non-GISTS. Tissue microarrays were used to detect the protein expression of DOG1 with the mouse antibodies MX067 and K9,rabbit antibody SP31,respectively;the results were analyzed. Results The positive rates of DOG1 MX067,K9 and SP31 antibodies were 90. 14%( 64/71),88. 73%( 63/71) and 88. 73%( 63/71),respectively. There was no significant difference between DOG1 MX067 antibody and DOG1 K9 antibody and DOG1 SP31 antibody( P>0. 05). DOG1 SP31 showed 8. 82%( 3/34) positive staining in schwannomas while others were negative. The sensitivity of DOG1 MX067 were higher than that of K9 and SP31;the specificity and area under ROC curve of DOG1 MX067 were higher than those of K9 and SP31. Conclusion The DOG1 antibody of clone mx067 has strong specificity and high sensitivity. It has high diagnostic value and clinical application value for GIST, and it is recommended to promote its application in the pathological diagnosis of GIST.
