摘 要:目的探讨二维应变超声心动图预测老年肺癌患者术后心功能的价值。方法选取2016年1月至2019年1月在中国人民武装警察部队四川省总队医院治疗的肺癌术后患者48例为观察组,另选取同期接受手术治疗的肺部良性疾病患者48例为对照组,所有患者均接受二维应变超声心动图检查。比较2组患者前间隔、前壁、后间隔、后壁的舒张早期纵向应变率峰值(SRe)、舒张晚期纵向应变率峰值(SRa)及左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)、左心室后壁厚度(LVPW)和室间隔厚度(IVS)。结果观察组左心室16节段室壁SRe、SRa各项指标均明显低于对照组,LVESD、LVEDD、LVPW、IVS水平均明显高于对照组[(32±3)mm比(30±3)mm、(48.3±2.5)mm比(46.0±2.2)mm、(34.6±2.8)mm比(29.1±2.8)mm、(10.8±1.1)mm比(8.2±1.1)mm],差异均有统计学意义(均P<0.01)。结论二维应变超声心动图能定量、准确地评价老年肺癌患者术后心功能变化,为肺癌患者预后提供参考。Objective To explore the value of two-dimensional strain echocardiography in predicting cardiac function in elderly patients with lung cancer after operation. Methods From January 2016 to January2019,48 patients with lung cancer who had surgical treatment in Sichuan General Hospital of the Chinese People’s Armed Police Force were enrolled as observation group;48 patients with benign lung disease were enrolled as control group. Two-dimensional strain echocardiography was performed. Early diastolic peak longitudinal strain rate( SRe),late diastolic peak longitudinal strain rate( SRa),left ventricular end-systolic diameter( LVESD),left ventricular end-diastolic diameter( LVEDD),left ventricular posterior wall thickness( LVPW) and interventricular septal thickness( IVS) were analyzed. Results SRe and SRa of 16 segments of left ventricular wall in observation group were significantly lower than those in control group;LVESD,LVEDD,LAD and IVS in observation group were significantly higher than those in control group[( 32 ± 3) mm vs( 30 ± 3) mm,( 48. 3 ± 2. 5) mm vs( 46. 0 ±2. 2) mm,( 34. 6 ± 2. 8) mm vs( 29. 1 ± 2. 8) mm,( 10. 8 ± 1. 1) mm vs( 8. 2 ± 1. 1) mm,all P < 0. 01].Conclusion Two-dimensional strain echocardiography can quantitatively and accurately evaluate the change of cardiac function and provide prognostic reference in elderly patients with lung cancer after operation.
