摘 要:目的:探讨基于角色理论的心理护理干预在老年肺癌患者中的应用效果。方法:将80例患者采用随机数字表法分为对照组(39例)和观察组(41例),对照组给予常规护理和普通心理护理,观察组在此基础上进行基于角色理论的心理护理干预,比较两组干预前后卡式评分(Karnofsky performance score,KPS)、焦虑自评量表(Self-Rating Anxiety Scale,SAS)评分、抑郁自评量表(Self-rating depression scale,SDS)评分、健康调查简表(SF-36)评分以及患者满意情况。结果:KPS、SAS及SDS评分干预前后两组内比较,差异均有统计学意义(P<0.05);KPS、SAS和SDS评分干预后两组间比较,差异均有统计学意义(P<0.05)。SF-36量表各单项内容评分干预前后两组组内比较,差异有统计学意义(P<0.05);干预后两组组间比较差异也有统计学意义(P<0.05)。患者满意率观察组为95.12%(39/41),对照组为82.05%(32/39),两组比较差异有统计学意义(P<0.05)。结论:辅助使用基于角色理论的心理干预可促进老年肺癌患者适应角色转变,减轻患者焦虑、抑郁情绪,改善行为能力,提高生活质量。Objective: To explore application effects of psychological nursing intervention to the elderly patients with lung cancer based on role theory. Methods: Eighty patients were allocated to the control group(39 cases) and the observation group(41 cases) using random number table method, the control group accepted conventional care and common psychological care, and the observation group psychological nursing intervention based on role theory. To compare the scores of KPS, SAS, SDS and SF-36 and the patients′ satisfaction conditions before and after the intervention in both groups. Results: The difference had statistical meaning in KPS scores, SAS and SDS before and after the intervention in both groups(P<0.05);the difference showed statistical meaning in KPS, SAS and SDS between both groups after the intervention(P <0.05). The difference was statistically significant in the scores of single item of SF-36 scale before and after the intervention within two groups(P<0.05);the difference had statistical meaning between both groups after the intervention(P <0.05). The patients′ satisfaction degree of the observation group was 95.12%(39/41), higher than 82.05%(32/39) of the control group, and there was a significant difference(P<0.05). Conclusion: Psychological intervention based on role theory as adjunctive therapy could help the elderly patients adapt to role change, relieve anxiety and depression, improve behavioral ability and lift quality of life.
