摘 要:目的分析种植区GBR手术同时填加血小板浓缩物-浓缩生长因子(concentrated growth factors,CGF)对成骨时间影响。方法选取本科室2017年1月—2018年12月收治骨量不足的牙体缺失患者60例,采用双盲分组法将其分成常规组及联合组,每组30例,分别接受单纯引导骨组织再生术(guided bone regeneration,GBR)治疗、GBR联合血小板浓缩物-浓缩生长因子(CGF)治疗。比较两组患者骨密度、缺损再生及成骨时间、疼痛程度、临床疗效。结果术前两组患者骨密度比较差异无统计学意义(P>0.05),术后增高;其中联合组患者术后1个月、3个月、6个月骨密度高于常规组,差异有统计学意义(P<0.05)。联合组骨痂及骨性连接形成时间少于常规组,差异有统计学意义(P<0.05),植骨高度、成骨厚度高于常规组,差异有统计学意义(P<0.05)。常规组及联合组患者疼痛程度差异比较无统计学意义(P>0.05)。常规组及联合组患者临床疗效比较差异有统计学意义(P<0.05)。结论GBR联合CGF治疗骨量不足的牙体缺失能明显提升临床疗效,减少成骨时间,不增加手术疼痛,促进缺损再生。Objective To analyze the effect of GBR and the concentrated growth factors(CGF)on bone formation time during operation.Methods 60 patients with teeth loss due to bone deficiency,from January 2017 to December 2018,were selected and divided into the conventional group and the combined group by a double-blind grouping method,30 patients in each group,who were treated by guided bone regeneration alone(GBR)and GBR combined with platelet concentration-concentrated growth factor(CGF)respectively.Bone mineral density,defect regeneration and osteogenesis time,pain degree and clinical effect were compared between the two groups.Results There was no statistically significant difference in bone mineral density between the two groups before surgery(P>0.05),and the bone mineral density increased after surgery.The bone mineral density of the combined group was higher than that of the conventional group at 1,3 and 6 months after surgery,the difference was statistically significant(P<0.05).The formation time of callus and osseous junction in the combined group was less than that in the conventional group,the difference was statistically significant(P<0.05),and the height and thickness of bone graft were higher than that in the conventional group(P<0.05).There was no statistically significant difference in pain degree between the conventional group and the combined group(P>0.05).The difference of clinical efficacy between the conventional group and the combined group was statistically significant,the difference was statistically significant(P<0.05).Conclusion GBR combined with CGF can significantly improve the clinical effect,reduce the osteogenesis time,do not increase the pain of surgery,and promote the regeneration of defect.
