摘　　要：幽门螺杆菌(Hp)感染已成为全球最常见的慢性感染之一,与慢性胃炎、消化性溃疡、胃癌及其癌前病变等疾病密切相关。根除Hp对于这些疾病的预防和治疗都有着重要作用。我国人口众多、人群Hp感染率高、卫生资源相对缺乏,采取全面Hp筛查和治疗策略的时机尚不成熟。目前临床常用的Hp根除方案包括标准三联疗法、非铋剂四联疗法和铋剂四联疗法,各方案的根除率有所差异,其中铋剂四联疗法的根除率最高,被推荐作为首选治疗方案。然而Hp感染在成功根除后仍存在复发情况,且复发可能与多种因素有关。此文对Hp的根除方案、根除后复发率及复发危险因素的相关最新研究进展作一综述,以期为Hp感染的预防和治疗提供新思路。Helicobacter pylori(Hp)infection is one of the common chronic infection worldwide.It is associated with chronic gastritis,peptic ulcers,gastric cancer and preneoplastic lesions.Eradication of Hp plays an important role in the prevention and treatment of these diseases.However,due to large population,high infection rate of Hp and limited access to health care resources in China,it is not yet ripe to adopt a comprehensive Hp screening and treatment strategy.In clinics,Hp eradication regimens commonly include standard triple therapy,non-bismuth quadruple therapy and bismuth quadruple therapy.While the eradication rate varies from each regimen,bismuth quadruple therapy has the highest eradication rate and is commonly recommended as the first line therapy.Nevertheless,recurrence of Hp infection is possible due to different reasons.This overview focuses on recent progress in eradication regimens,recurrence rate and risk factors of Hp infection,which might provide new ideas for prevention and treatment of Hp infection.
