摘 要:目的评价光动力疗法(PDT)治疗种植体周围炎的临床效果。方法采用Cochrane系统评价方法,检索EMBASE、Cochrane图书馆、Medline/PubMed、CNKI和CBM等数据库中有关PDT治疗种植体周围炎的临床随机对照试验,检索时间截止至2019年4月30日,提取纳入研究的相关资料并进行质量评价,采用RevMan5.3软件对结果进行Meta分析。结果共纳入5个临床随机对照试验进行合并分析,结果显示PDT辅助机械洁治在改善种植体的探诊深度、探诊出血和菌斑指数方面与对照组比较差异均具有统计学意义(P<0.05),但是在改善种植体的临床附着水平和边缘骨水平方面与对照组比较,差异均无统计学意义(P>0.05);同时,在术后6个月至1年的观察中,以上各指标在试验组与对照组之间比较差异均无统计学意义(P>0.05)。结论 PDT治疗种植体周围炎在改善种植体的探诊深度、探诊出血和菌斑指数等指标方面能够获得一定疗效,但从长期疗效来看,PDT联合机械洁治在治疗种植体周围炎方面并不能获得优于单纯机械洁治以及药物治疗的效果。Objective To systematically evaluate the efficacy of photodynamic therapy(PDT) in the treatment of peri-implantitis.Methods The EMBASE, Cochrane library, Medline/Pub Med, CNKI and CBM databases were searched for clinical randomized controlled trials about PDT for the treatment of peri-implantitis up to 30 April 2019. The data extraction and quality assessment were carried out. The Rev Man 5.3 was applied for meta-analysis.Results A total of 5 randomized controlled trials were included for combined analysis. The results showed that compared with the control group, PDT assisted mechanical debridement had significant differences in improving the probing depths, bleeding on probing and plaque index(P<0.05), but had no significant effect on improving the clinical attachment level and marginal bone level of implants(P>0.05). Besides, all the clinical outcomes showed no statistical difference between the experimental group and the control group after 6 months to 1 year of the therapy(P>0.05). Conclusion PDT is effective in improving the probing depths, bleeding on probing and plaque index of peri-implantitis. However, in terms of long-term efficacy, PDT combined with mechanical debridement cannot achieve better results than single mechanical debridement or drug treatment in the treatment of peri-implantitis. More randomized clinical trials with large sample size and high quality are required to assess the efficacy of PDT for the treatment of peri-implantitis.
