摘  要:目的探讨血管和子宫基质金属蛋白酶-1(MMP-1)、基质金属蛋白酶-7(MMP-7)等级和Ⅰ型胶原沉积与妊娠期高血压的关系。方法选取2014年1月至2018年8月收治的妊娠期高血压患者100例为试验组,均为妊娠期20周后血压≥140/90 mm Hg(2次测量间隔至少6 h)并无蛋白尿(24 h>300 mg或一次尿试验至少2次)的孕妇。另选取同期收治的100例健康孕妇为对照组。还包括试验组及对照组产妇分娩后的新生儿各100例。采用酶联免疫吸附法检测200例孕妇血清中MMP-1和MMP-7的含量;采用RFLP-PCR方法检测16071G/2G MMP-1、735 C/T MMP-2、1306 C/T MMP-2、11715A/6A MMP-3和1562C/T MMP-9基因多态性的分布。结果MMP-1和MMP-7在试验组高表达(P=0.085),TIMP-1和TIMP-2表达较低。试验组孕妇年龄与TIMP-1呈弱正相关(r=0.278,P<0.05),对照组孕周与TIMP-2呈弱正相关(r=0.318,P<0.05)。结论血管和子宫MMP-1、MMP-7可能参与了妊娠期高血压的病理生理过程,进一步评价MMP-1和MMP-7作为预测孕妇妊娠期高血压的潜在生物标志物的可能价值。Objective To investigate the relationship between matrix metalloproteinase-1/-7 grade and type I collagen in blood vessel and uterus with pregnancy-induced hypertension.Methods A total of 100 patients with pregnancy-induced hypertension who were admitted and treated in our hospital from January 2014 to August 2018 were enrolled as experimental group,who were the pregnant women with blood pressure being higher than 140/90 mm Hg after 20 weeks gestation,without proteinuria.Meanwhile the other 100 healthy pregnant women who were admitted in our hospital were enrolled as control group,moreover,the 100 newborns in experimental group and 100 newborns in control group were enrolled in the study.The serum levels of MMP-1 and MMP-7 in the 200 pregnant women were detected by ELISA.The distribution of 16071G/2G MMP-1,735C/T MMP-2,1306 C/T MMP-2,11715A/6A MMP-3 and 1562C/T MMP-9 gene polymorphism were detected by RFLP PCR.Results The MMP-1 and MMP-7 were highly expressed in experimental group,however the expression levels of TIMP-1 and TIMP-2 were lower.There was a weak positive correlation between the age of pregnant women and TIMP-1 in experimental group(r=0.278,P<0.05),moreover there was a weak positive correlation between the gestational weeks and TIMP-2 in control group(r=0.318,P<0.05).Conclusion The matrix metalloproteinases 1,7 in blood vessel and uterus of pregnant women may be involved in the pathophysiological and physiological process of pregnancy-induced hypertension.It is necessary to evaluate further the possible value of MMP-1 and MMP-7 as the potential biomarker for predicting pregnancy-induced hypertension.
