CEA、AFP、CA199、CA242、CA724对胃癌诊断及预后的评估价值

摘　　要：目的探讨癌胚抗原(CEA)、甲胎蛋白(AFP)、唾液酸化岩藻戊糖(CA199)、糖蛋白抗原242(CA242)、糖蛋白抗原724(CA724)对胃癌诊断及预后的评估价值。方法选取收治的胃癌患者216例为观察组,同时以同期进行健康体检的健康者216例为对照组。检测2组患者血清CEA、AFP、CA199、CA242、CA724水平,并对检测结果进行比较;比较观察组中不同分期患者的肿瘤标志物水平;比较肿瘤标志物检测结果的诊断效果;并对胃癌患者的预后影响因素进行分析。结果观察组患者各项肿瘤标志物水平均显著高于对照组(P<0.05);不同分期的胃癌患者各项肿瘤标志物水平比较,Ⅰ+Ⅱ期者均明显低于Ⅲ+Ⅳ者(P<0.05);胃癌Ⅰ、Ⅱ、Ⅲ期患者采用各肿瘤标志物联合检测的阳性率显著高于各项肿瘤标志物单项检测(P<0.05);但胃癌Ⅳ期患者的检测阳性率差异无统计学意义(P>0.05);本组216例患者,随访3年,共死亡96例,Cox逐步回归分析显示,术前CEA浓度、淋巴结转移为影响胃癌患者3年生存期的重要因素。结论CEA、AFP、CA199、CA242、CA724联合检测在提高胃癌检出率、预测患者预后方面可发挥积极作用,值得推广应用。Objective To investigate the significance of carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP),CA199,glycoprotein antigen 242(CA242),and glycoprotein antigen 724(CA724)in the diagnosis and prognosis evaluation of gastric cancer.Methods A total of 216 patients with gastric cancer were enrolled in the observation group,and 216 healthy subjects who underwent physical examination during the same period were selected as the control group.After obtaining the informed consent of the patients,the levels of serum CEA,AFP,CA199,CA242,and CA724 were detected in two groups,and the results were compared.The tumor markers in patients with different stages of gastric cancer were compared in the observation group.The diagnostic results of tumor markers were compared,and the prognostic factors for gastric cancer were analyzed.Results The levels of tumor markers in observation group were significantly higher than those in control group(P<0.05).In terms of different stages of gastric cancer,the tumor markers in patients with stageⅢ+Ⅳwere significantly lower than those with stageⅢ+Ⅳ(P<0.05).The positive rate of combined detection of tumor markers in patients with stageⅠ,ⅡandⅢgastric cancer was significantly higher than that of single tumor markers(P<0.05).There was no significant difference in the positive rate of detection in patients with stageⅣgastric cancer(P>0.05).In this group,216 patients were followed up for 3 years,and 96 patients died.Cox stepwise regression analysis showed that preoperative CEA concentration and lymph node metastasis were important factors affecting the 3-year survival of gastric cancer patients.Conclusion The combined detection of CEA,AFP,CA199,CA242 and CA724 can play an active role in improving the detection rate of gastric cancer and predicting the prognosis of patients,which is,therefore,worthy of popularization and application.
