摘 要:目的探讨根管显微镜(DOM)在检测上颌第一磨牙近中颊根第二根管(MB2)中的临床价值。方法收集98颗上颌第一磨牙离体牙为实验对象,分别使用DOM及锥体束CT探查MB2。比较两种方法对MB2的检测情况;并将锥体束CT诊断结果作为参照,评价DOM对MB2的检测效能。结果 DOM和锥形束CT分别在66颗、68颗离体牙中检出MB2,检出率分别为67.3%、69.4%,差异无统计学意义(P>0.05),两种方法的检出结果具有较好的一致性(Kappa值=0.670,P<0.05)。DOM检测MB2的灵敏度和特异度分别为90.9%、75.0%。结论 DOM有助于发现并定位上颌第一磨牙MB2。Objective To explore the clinical value of dental operating microscope(DOM) for detecting the second mesiobuccal canal(MB2) in the maxillary first molar. Methods Ninety-eight extracted maxillary first molars were collected as experimental subjects, and then the MB2 was detected by DOM and cone-bean CT separately. The detection for MB2 by the two methods was compared;besides, the efficacy of DOM for detecting MB2 was assessed with the diagnostic results of cone-bean CT as a reference. Results MB2 was found in 66 extracted teeth and 68 extracted teeth by DOM and cone-bean CT respectively, but there was no statistically significant difference in detection rate between DOM and cone-bean CT(67.3% vs. 69.4%, P>0.05), and a favorable consistency was observed in detection results between the two methods(Kappa value=0.670,P<0.05). The sensitivity and specificity of DOM for detecting MB2 were 90.9% and 75.0% respectively. Conclusion DOM is beneficial to the detection and positioning of MB2 in the maxillary first molar.
