摘　　要：目的探讨三氧化矿物凝聚体(MTA)应用于显微根尖手术治疗难治性慢性根尖周炎的临床效果。方法选取我院2017年4月至2019年4月收治的685例难治性慢性根尖周炎患者作为研究对象,采用抓阄法将其随机分为观察组(341例,MTA+显微根尖手术)和对照组(344例,显微根尖手术)。比较两组患者的治疗效果、疼痛程度、肿痛情况及旧根尖周指数(O-PAI)评分。结果两组患者的治疗总有效率比较,差异无统计学意义(P>0.05)。术后5 d,两组患者的Negm评分均降低,且观察组低于对照组(P<0.05)。观察组的肿痛率低于对照组(P<0.05)。术后1、3个月,两组的O-PAI评分均低于术前,且观察组低于对照组(P<0.05)。结论MTA应用于显微根尖手术治疗难治性慢性根尖周炎的临床效果显著,能够减轻肿痛感,值得临床进一步推广应用。Objective To explore the clinical effect of mineral trioxide aggregate(MTA)applied in microscopic apical surgery for refractory chronic apical periodontitis.Methods A total of 685 patients with refractory chronic apical periodontitis admitted in our hospital from April 2017 to April 2019 were selected as the research objects,and they were randomly divided into observation group(341 cases,MTA+microscopic apical surgery)and control group(344 cases,microscopic apical surgery)by grasping method.The treatment effect,pain degree,swelling and pain and old periapical index(O-PAI)score of the two groups were compared.Results There was no significant difference in the total effective rate of treatment between the two groups(P>0.05).On the 5 th day after operation,Negm score in the two groups decreased,and that in the observation group was lower than the control group(P<0.05).The swelling and pain rate in the observation group was lower than that in the control group(P<0.05).At 1 and 3 months after operation,the O-PAI scores in the two groups were lower than those before operation,and those in the observation group were lower than the control group(P<0.05).Conclusion MTA applied in microscopic apical surgery for refractory chronic apical periodontitis has a significant clinical effect,it can reduce the swelling and pain,which is worthy of further clinical promotion and application.
